      目的 探讨不同菌型幽门螺杆菌(Hp)感染消化性溃疡(PU)患者血清中T细胞亚群的变化以及白细胞介素(IL)-17、转化生长因子(TGF)-β的表达。方法 随机选择经胃镜检查确诊的PU患者共88例,14C或13C标记尿素呼气试验检测Hp感染状况,血清Hp CagA、VacA、UreB、UreA等4项基因免疫印迹法检测Ⅰ型Hp和Ⅱ型Hp感染情况;同期纳入30例健康体检者作为对照组;采用流式细胞仪检测所有研究对象外周血T 淋巴细胞亚群和NK 细胞的活性;Elisa法测定患者血清白细胞介素(IL)-17、转化生长因子(TGF)-β含量。结果 Ⅰ型Hp组和Ⅱ型Hp组患者CD4 和IL-17水平明显高于Hp阴性组,而TGF-β水平明显低于Hp阴性组(P<0.01);Ⅱ型Hp组患者CD4 和IL-17水平明显低于Ⅰ型Hp组,TGF-β水平明显高于Ⅰ型Hp组(P<0.01);在Ⅰ型Hp感染组,十二指肠球部溃疡(DU)患者的CD4 和IL-17表达水平明显高于胃溃疡(GU)患者,而TGF-β水平明显低于GU患者(P <0.01);在Ⅱ型Hp组,CD4 、IL-17和TGF-β水平在DU与GU间的表达水平差异无显著差异(P >0.05);结论 血清CD4 T细胞增高及其细胞因子IL-17和TGF-β的表达异常可能在Hp感染PU中发挥了重要调节作用。

      【】 Objective To investigate change of serum T cell subset in peptic ulcer(PU) with different types bacteria helicobacter pylori(Hp) infection and expression of interleukin(IL)-17 and transforming growth factors(TGF)-β. Methods 88 patients with PU confirmed by gastroscope examination for the study,while at the same time 30 healthy examined people as control group.Hp infection was detected by 14 c or 13 c urea breath test and Hp infection types were detected by immunoblotting of serum Hp CagA,VacA,UreB and UreA.Peripheral blood T lymphocyte subsets and NK cells activities were detected by flow cytometer in all cases. Interleukin(IL)-17 and transforming growth factors(TGF)-β in serum were detected by elisa method. Results CD4+ and IL-17 of Hp type Ⅰ and Hp type Ⅱ groups were obviously higher than Hp negative group,while TGF-β was evidently lower(P<0.01).CD4+ and IL-17 of Hp type Ⅱ group were obviously lower than Hp Ⅰ group,while TGF-β was evidently higher(P<0.01). In Hp type Ⅰ group,CD4+ and IL-17 of duodenal ulcer(DU) were obviously higher than gastric ulcer(GU),while TGF-β was evidently lower(P<0.05).In Hp type Ⅱ group,CD4+,IL-17 and TGF-β in DU and GU had no significant differences(P>0.05). Conclusions Increasing of serum CD4+ T cell and abnormal expression of IL-17 and TGF-β may play an important regulating role in PU with Hp infection.
