| 摘要：目的 观察吡酮莫特治疗慢性鼻窦炎的疗效以及对血清白细胞介素（IL）-8和IL-13的影响。方法 98例慢性鼻窦炎均为本院2013年2月～2015年2月间收治病例，参照数字表法随机分为对照组和观察组，两组均49例；对照组参照《慢性鼻?鼻窦炎诊断和治疗指南》进行治疗；鼻腔冲洗：采用0.9％氯化钠注射液500mL冲洗鼻腔，2次/d。持续严重鼻塞者可使用0.5％呋嘛滴鼻液，2滴/次，3次/d，疗程＜7d；口服克拉霉素片，0.25g/次，1次/12h；观察组在对照组用药基础上口服匹多莫德颗粒，开始2周0.4g/次，2次/d；后2周0.4g/次，1次/d。两组均连续治疗4周。比较两组视觉模拟量表（VAS）评分、鼻窦CT Lund-Mackay评分、鼻内镜Lund-Kennedy评分；检测两组血清中IL-8和IL-13水平。结果 观察组治疗后2周和4周患者VAS评分、鼻窦CT Lund-Mackay评分以及鼻内镜Lund-Kennedy评分均明显少于同期对照组（P＜0.01）。观察组治疗后患者血清IL-8和IL-13水平均明显低于对照组，比较差异有统计学意义（P＜0.01）。结论 吡酮莫特治疗慢性鼻窦炎的疗效显著，抑制患者血清IL-8和IL-13水平可能为其作用机制之一。 |

| Objective To observe efficacy of pyrrone mulder in treatment of chronic nasosinusitis and mechanism analysis and its influence on interleukin(IL)-8 and IL-13.Methods 98 patients with chronic nasosinusitis in our hospital from february of 2013 to february of 2015 were randomly divided into control group and observation group with 49 cases of each group according to the random number table method.Control group was treated referring to “diagnostic and therapeutic guidelines of chronic nasosinusitis”,nasal douche with 500 mL 0.9% sodium chloride injection and bid,severe nasal obstruction with 0.5% ephedrine nasal drops with 2 drops/time and tid and within 7 ds,and taking orally clarithromycin tablets with 0.25g/time and 1time/12h.Observation group was given wity Pi do Maude granule,0.4g/time and 2times/d at first two weeks,0.4g/time and 1 time/d at second two weeks.Courses were for 3 weeks in both groups.Scores of visual analogue scale,paranasal sinus CT Lund-Mackay,and Nasal endoscopy Lund-Kennedy were compared between both groups.Expression levels of IL-8 and IL-13 between two groups were detected. Results After 2 and 4 weeks’treatment,scores of visual analogue scale,paranasal sinus CT Lund-Mackay,and Nasal endoscopy Lund-Kennedy in observation group were obviously lower than those of control group(P＜0.01).Serum levels of IL-8 and IL-13 of observation group were remarkably lower than control one after treatmentwith statistic difference(P＜0.01).Conclusion Pyrrone mulder in treating chronic nasosinusitis could be effective in treatment,and inhibiting serum levels of IL-8 and IL-13 may be one of mechanisms. |
