      目的 检测丹参酮ⅡA磺酸钠灌流对急性心肌梗死大鼠离体心脏心肌组织间液钙离子(Ca2+)及心肌肌钙蛋白T(cardiac troponin T, cTnT) 浓度的影响,分析其影响心脏做功的机制。方法 将40只SD大鼠随机分为模型组、普萘洛尔组、异丙肾组、丹参酮组,采用舌下静脉注射垂体后叶素的方法建立大鼠急性心肌梗死动物模型,于造模2 h后断头取心,固定于 Langendorff离体心脏灌流装置后灌流给药。分别检测用药前10 min及药后0.5、1 h心肌组织间液Ca2+及cTnT 浓度,心肌收缩力(isometric tension,IT)、心率(reart rate,HR)、左室最大上升/下降速率(±LVdp/dtmax)、左室射血分数(LVEF)、左室内压力峰值(LVSP)。结果 丹参酮组在灌流丹参酮ⅡA磺酸钠用药后0.5和1 h时,cTnT、IT、±LVdp/dtmax、LVEF、LVSP明显降低,组织间液中Ca2+明显升高,与用药前及异丙肾组比较, P<0.05,但HR实验前后差异无显著性。结论 急性心肌梗死时存在钙超载,其机制是大量Ca2+内流与cTnT结合,cTnT浓度增高、心肌组织间液Ca2+降低,而丹参酮ⅡA磺酸钠可阻止Ca2+内流,减少心肌组织间液Ca2+过量内流造成钙超载,降低心脏做功,避免钙超载诱导和加重急性心肌梗死。

      Objective To investigate the perfusion effect of sodium tanshinone IIA sulfonate on the concentrations of calcium ion (Ca2+) of myocardial interstitial liquid and cardiac troponin T (cTnT) in the heart isolated from rats with acute myocardial infarction (AMI) ,and also analyze the underlying mechanism of its influence on cardiac work. Methods 40 SD rats were divided into model group,propranolol group,isoproterenol group,and tanshinone group. The model of acute myocardial infarction was established by sublingual venous injection of pituitrin .Two hours after model establishment,decapitation was carried out and the hearts were taken out and treated with perfusion after fixed in the Langendorff isolated heart perfusion device. Concentrations of Ca2+ ,and cTnT,HR,Isometric tension (IT),heart rate (±LVdp/dtmax),left ventricular maximal rise/fall rate (LVEF),and left ventricular pressure peak (LVSP) were recorded 10 min before treatment and 0.5 h,and 1 h after treatment,,respectively. Results In the tanshinone group,cTnT,IT,±LV dp/dtmax,LVEF and LVSP were significantly lower 0.5 h and 1 h after the treatment,while Ca2+ concentration was significantly higher compared to that before treatment and the isoproterenol group(P<0.05). However,no significant difference was observed before and after treatment with respect to HR. Conclusion Calcium overload was observed during acute myocardial infarction ,which was resulted from a large influx of Ca2+ and its combination with cTnT,leading to increased concentrations of cTnT and decreased concentrations of Ca2+ in myocardial interstitial fluid .Sodium tanshinone IIA sulfonate was found to prevent Ca2+ influx,reduce calcium overload in myocardial interstitial fluid,and also reduce cardiac work ,thereby avoiding calcium overload induction and exacerbation of AMI.
