      目的 评价烧伤变形脱细胞真皮基质(denatured acellular dermal matrix,DADM)用于烧伤创面修复的可行性。方法 选取该院收治的80例烧伤患者为研究对象,按照随机数字表法,分为研究组和对照组,每组40例。实验组患者实施DADM覆盖创面移植术,对照组实施异种脱细胞真皮基质(acellular dermal matrix,ADM)覆盖创面移植术。比较两组患者的植皮成活率、瘢痕形成及治疗前后主要实验室指标。结果 烧伤变形脱细胞真皮基质的疗效显著强于异种脱细胞真皮基质(χ2=3.152,P=0.002)。烧伤变形脱细胞真皮基质较异种脱细胞真皮基质能够减轻瘢痕增生,研究组的瘢痕评分显著高于对照组(χ2=9.241,P=0.002)),其中愈合时间(18.2±5.76 vs 30.2±6.54)d,血管情况得分(0.8±0.78 vs 1.5±1.21),柔软性(0.9±0.92 vs 1.6±0.98),色素情况(0.7±0.98 vs 1.4±1.13),高度(0.4±0.35 vs 0.9±0.72)。治疗1周及3周后,研究组患者的白介素6(IL-6)及干扰素γ(IFN-γ)水平显著低于对照组,白介素2(IL-2)水平及CD4/CD8比值显著高于对照组,两组差异均有统计学意义(P<0.05)。结论 烧伤变形脱细胞真皮基质可作为真皮替代物,用于烧伤创面的修复,能够减轻瘢痕增生,值得推广应用。

      Objective To evaluate the feasibility of denatured acellular dermal matrix for burn wound repair.Methods 80 cases were chosen from February 2014 to February 2015 in our hospital as research objects and they were randomly divided into the study group (n=40) and the control group (n=40).In the study group,the patients were engrafted with denatured acellular dermal matrix.In the control group,the patients were engrafted with heterogeneity acellular dermal matrix.The survival rate of engrafts,scar hyperplasy and laboratory index were determined after treatment in both groups.Results The curative effect of denatured acellular dermal matrix was much better than acellular dermal matrix(χ2=3.152,P=0.002).The scar hyperplasia under DADM treatment was much better than that under ADM,the scar scores in the study group were significantly higher than the control group(χ2=9.241,P=0.002),[healing time (18.2±5.76 vs 30.2 ±6.54),vascular condition score (0.8±0.78 vs 1.5±1.21),flexibility (0.9±0.92 vs 1.6±0.98),pigment (0.7±0.98 vs 1.4±1.13),and height (0.4±0.35 vs 0.9±0.72)].After 1 week and 3 weeks treatment,the levels of IL-6 and IFN-γ were significantly lower than the control group,and the levels of IL-2 and CD4 / CD8 ratio were significantly higher than the control group (P<0.05).Conclusions Denatured acellular dermal matrix could serve as dermal template to repair the burn wound,thereby relieving scar proliferation,and it is worthy applying in the future.
