      目的 探讨2型糖尿病患者血尿酸(BUA)、尿微量白蛋白(UAER)与大血管病变的关系。方法 以该院收治的100例2型糖尿病患者为研究对象,其中出现大血管病变53例,计为观察组,无大血管病变47例,计为常规组,另选择同期50例行健康体检者为对照组,比较三组血糖[空腹血糖(FPG)、糖化血红蛋白(HbAlC)]、血压、血脂[总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)]、BUA及UAER水平。结果 与对照组比较,观察组、常规组患者收缩压、FPG、HbAlC、TC、TG、LDL-C水平明显高,差异有统计学意义(P<0.05)。与常规组比较,观察组HbA1C、LDL-C水平明显高,差异有统计学意义(P<0.05)。三组血BUA[(362.5±112.0)vs(284.7±68.5)vs(180.4±103.0)]μmol·L-1、UAER[(180.3±22.9)vs(68.2±12.2)vs(21.0±8.4)]mg/24 h比较差异均有统计学意义(P<0.05)。结论 2型糖尿病大血管病变患者表现出高血BUA、高UAER特点,可将血BUA、UAER水平作为2型糖尿病大血管病变风险预测因子之一。

      Objective To investigate the relationship between blood uric acid (BUA),urinary albumin excretion rate (UAER) levels and macroangiopathy in patients with type 2 diabetes mellitus.Methods 100 cases of patients with type 2 diabetes mellitus admitted in our hospital between February 2013 and October 2015 were involved in the study,including 53 cases with macroangiopathy (complication group) and 47 cases without (routine group).At the same time,50 cases of healthy people who underwent physical examination were selected as the control group.The levels of blood glucose [fasting blood glucose (FBG),glycosylated hemoglobin (HbAlc)],blood pressure,blood lipid [total cholesterol (TC),triglyceride (TG),high density lipoprotein cholesterol (HDL-C),low density lipoprotein cholesterin (LDL-C)],BUA and UAER were compared among the three groups.Results The systolic blood pressure,levels of FPG,HbAlc,TC,TG and LDL-C of the complication group and the routine group were significantly higher than t the control group (P<0.05).The levels of HbAlC and LDL-C in the complication group were significantly higher than those in the routine group (P<0.05).There were significant differences in BUA [(362.5±112.0) vs (284.7±68.5) vs (180.4±103.0)] μmol·L-1 and UAER [(180.3±22.9) vs (68.2±12.2) vs (21.0± 8.4)] mg/24 h among the three groups (P<0.05).Conclusions Type 2 diabetic patients with macroangiopathy showed high BUA and UAER.BUA and UAER levels can be used as one of the risk predictors of macroangiopathy in type 2 diabetes mellitus.
