| 目的 比较发育性髋关节脱位不同脱位程度接受一期手术后的近、中期结果。方法 对652例(共864髋)接受一期手术的DDH患者的临床资料进行回顾性分析,根据患者术前脱位程度(Tonnis标准)进行分组,通过临床表现(Mckay标准)和影像结果(Severin方法)进行评价和比较。结果 平均随访的时间为6.2年(3.2~8.9年),根据McKay临床功能评价术后优秀或良好的髋占79.4％,根据Severin影像学分型,术后优秀或良好的占84.7％,术前脱位程度为TonnisⅡ、Ⅲ度的髋出现32例再脱位或残余髋臼发育不良。结论 术前脱位程度为TonnisⅡ度和Ⅲ度的患者术后再脱位和残余髋臼发育不良的发生率明显较高。 |

| Objective To compare the recent and future therapeutic effect outcomes among different degrees of developmental dislocation of the hip after one-stage surgical treatment.Methods We retrospectively reviewed 652 patients with a total of 864 hip joints.The patients were assigned into different groups according to the degree of preoperative dislocation(Tonnis standard).The clinical(Mckay criteria) and radiographic outcomes(Severin’s radiographic criteria) were evaluated and compared.Results The average time of follow-up was 6.2 years(range:3.2~8.9 years).A total of 79.4% of good or excellent outcomes were obtained for clinical functional evaluation according to the McKay criteria,good or excellent accounting for 84.7% according to the Severin’s radiographic criteria.There were 32 cases of redislocation or residual acetabular dysplasia in the degree of preoperative dislocation of Tonnis Ⅱ and Ⅲ hip dislocation.Conclusions A significantly higher incidence of redislocation and residual acetabular dysplasia was observed in Tonnis grade II and III hip dislocation. |
