| 目的 比较三种抗精神病药物治疗初诊精神分裂症患者的临床效果及对患者服药依从性、不良反应发生率的影响。方法 选取90例初诊精神分裂症患者作为研究对象,按随机数字表法分为A、B、C三组,每组30例。分别采用奥氮平、利培酮、阿立哌唑治疗,比较三种抗精神病药物治疗初诊精神分裂症的效果及安全性。结果(1)临床症状:治疗前,三组阳性和阴性症状评分(PANSS)量表总分对比差异均无统计学意义(P>0.05)；治疗6个月后,A、B、C三组阳性、阴性、病理学评分较治疗前降低(A组t=5.915、3.636、4.243、4.646；B组t=6.462、3.096、6.696、5.512；C组t=4.093、3.059、4.206、3.868,P均<0.05),但三组治疗6个月后对比无统计学意义(P>0.05)；(2)服药依从性及社会功能:治疗前,三组个人与社会功能(PSP)表及服药态度(DAI)量表评分对比差异无统计学意义(P>0.05)；治疗后6个月,A、B、C三组PSP评分均明显上升,与治疗前对比差异有统计学意义(t=3.197、3.005、5.4994,P<0.05),但三组组间对比差异无统计学意义(P>0.05)；三组治疗后DAI评分均上升,与治疗前对比差异有统计学意义(t=2.557、5.206、2.934,P<0.05),但组间对比差异无统计学意义(P>0.05)；(3)不良反应:静坐不能为三组常见不良反应,发生率分别为30.00%、23.33%、26.67%；B组乏力发生率为43.33%,高于C组(P<0.05)；A组肌张力障碍发生率高于B、C组(P<0.05)；B组口干、便秘、嗜睡发生率均高于A、C组(P<0.05)。结论 三种抗精神病药物对初诊精神分裂症患者阳性、阴性症状均有明显的改善作用,以奥氮平在病理学改善方面更为显著,但三种药物在服药依从性及患者社会功能的改善方面无明显差异,在不良反应方面,以阿立哌唑不良反应发生率较低。 |

| Objective To compare the clinical effects amongthree kinds of antipsychotic drugs in the treatment of patients with newly diagnosed schizophrenia and the effects on medication complianceand the incidence of adverse reactions.Methods Ninety patients with newly diagnosed schizophrenia who were treated in The Second Affiliated Hospital of Xinxiang Medical Universitybetween February 2012 and February 2015 weretreated as the research objects.According to the random number table method,theywereassignedinto group A,group Bandgroup C,with 30 cases in each group.The three groups were treated by olanzapine,risperidone andaripiprazole respectively.The effects and safety were compared among the three kinds of antipsychotic drugs.Results (1)As for clinical symptoms,there were no statistically significant differences in PANSS scores among the three groups before treatment(P> 0.05).Sixmonths after treatment,the positive,negative and pathological scores of group A,B and Cwere lower than those before treatment(group A t=5.915,3.636,4.243,4.646;group B t=6.462,3.096,6.696,5.512;group C t=4.093,3.059,4.206,3.868,all P<0.05)but there were no differences among the three groups 6 months after treatment(P>0.05);(2)As for medication compliance and social function,there were no statistical differences in the scores of PSP and DAI among the three groups before treatment(P>0.05);6 months after treatment,PSP scores of group A,B and Cincreased significantly.Compared with those before treatment,there were statistically significant differences(t=3.197,3.005,5.4994,P<0.05)but there was no significant difference among the three groups(P>0.05);After treatment,the DAI scores of the three groups all increased.Compared with those before treatment,there were statistically significant differences(t=2.557,5.206,2.934,P<0.05),but there was no significant difference amongthe groups(P>0.05).(3)As for adverse reactions,akathisiawas the common adverse reaction in the three groups and the incidence rates in the three groups were 30.00%,23.33% and 26.67% respectively.The incidence of weakness in group B(43.33%)was higher than that in group C (P<0.05).The incidence rate of dystonia in group Awas higher than thosein group Band C(P < 0.05).The incidence rates of dry mouth,constipation,drowsiness in group Bwere higher than thosein group Aand C(P< 0.05).Conclusions The three kinds of antipsychotic drugs are of significant effects in improving positive and negative symptoms of patients with newly diagnosed schizophrenia and the effect of olanzapine in pathological improvementis more significant,but there are no significant differences in improvement of medication compliance and the social function ofpatientsamongthe three kinds of drugs.In terms of adverse reactions,the incidence of adverse reactions of aripiprazole is relatively lower. |
