| 目的 回顾性分析某院2013—2015年门诊处方和住院患者抗菌药物使用的情况,提高该院门诊及住院患者抗菌药物使用的合理性和规范性。方法 按照全国抗菌药物临床应用监测网的上报要求,随机抽查2013—2015年每月11~20日出院的非手术和手术病历各15份,3年共1 080份；抽查每月16日门诊成人普通处方100张,3年共3 600张；统计每季度出院患者抗菌药物品种、消耗量、使用金额及抗菌药物使用强度。结果 该院抽查的门诊处方抗菌药物使用率基本低于20%,抗菌药物使用基本合理；住院患者抗菌药物使用率低于60%,I类切口围手术期预防使用率降至30%,给药时机和药物使用天数基本合理。结论 该院抗菌药物的使用基本合理,但住院患者使用抗菌药物也存在一些问题,需要继续增强临床医生合理使用抗菌药物的意识,加强抗菌药物使用的督导,促进抗菌药物的安全、有效、经济地使用。 |

| Objective To retrospectively analyze outpatient prescriptions and the use of antimicrobial agents in hospitalized patients in 2013—2015 and to improve the rationality of the application of antimicrobial agents in a hospital.Methods According to the manual for Antibacterial Surveillance Center,respective 15 copies of medical records of non-surgical and surgical patients were extracted randomly who were discharged from the hospital during 11~20 every month between 2013—2015 and 1080 copies were analyzed;100 cases of outpatient prescriptions for adult were extracted in the 16th every month,totally 3600 copies collected during 2013—2015.The types,consumption,amount and defined daily dose of antibacterial drugs of discharged patients were calculated every quarter of the year in 2013—2015.Results The rate of antimicrobial agent use among outpatients was mainly less than 20% and the use of antimicrobial agents was mostly reasonable.The rate of antimicrobial agent use of inpatients was less than 60%.The prophylactic use rate of antimicrobial agents during perioperative period for type Iincision declined to 30%.Meanwhile,the opportunity and length of use of antimicrobial agents were rational.Conclusions The data statistics and analysis about antibacterials surveillance center of the hospital in 2013—2015 showed a rational use of antibacterial drugs.But irrational use still exists in some operations.Therefore,doctors should improve their awareness about rational use of antibacterial drugs and the supervision of antibacterial use is needed to enhance to promote the safe,effective and economic use of antibacterial agents. |
