| 目的 探讨浓度为40%、60%的笑气吸入清醒镇静法结合高速气涡轮手机在牙科焦虑症患者下颌阻生第三磨牙拔除术中的临床效果。 方法 拔除下颌阻生第三磨牙牙科焦虑症患者120例,用简单随机法分为三组,每组40例:A组(40%笑气浓度+局麻),B组(60%笑气浓度+局麻),C组对照组只在局麻下拔牙。拔牙过程中三组患者均在心电监护仪下监测心率和血氧饱和度,观察记录手术前、手术中、手术后各项生命体征变化,对各组患者进行视觉模拟疼痛评分(VAS)以及Ramsay镇静评分,并对结果进行统计分析。 结果 试验组和对照组在手术过程中,生命体征变化评估以及镇静镇痛的效果比较均差异有统计学意义(P＜0.05)。 结论 笑气吸入清醒镇静技术具有减轻焦虑、镇痛镇静作用,能使患者更好地配合治疗,且60%笑气浓度较40%效果更佳,值得临床推广应用。 |

| ObjectiveTo evaluate the application of inhaled nitrous oxide and handpiece in the extraction of mandibular impacted molar in dental anxiety patients. Methods 120 cases with impacted mandibular wisdom tooth extraction were randomly divided into three groups:A(40% N2O+local anesthesia),B(60% N2O+ local anesthesia),C (control group,local anesthesia).The changes of heart rate and oxygen saturation in the extraction process were recorded.The pain level and sedation effects were analyzed with visual analogue scale (VAS) and Ramsay scale.The parameters were statistically analysed. Results Nitrous oxide inhalation group showed much less changes in heart rate and blood pressure；much lower VAS pain score and Ramsay score than local anesthesia group.There were significant difference between them (P＜0.05). Conclusions Nitrous oxide inhalation is effective in alleviating dental anxiety and it has a good sedative and analgesic effects during impacted wisdom tooth extraction.thus worth wide clinical use. |
