| 目的 了解某院伏立康唑预防/治疗侵袭性真菌病的临床应用情况,分析伏立康唑的用药合理性。方法 对2016年1月1日—2016年12月31日期间使用伏立康唑的住院患者信息进行回顾性统计分析,根据制定的合理性评价标准,对伏立康唑使用情况进行合理性评价。结果 期间共计有425例至少使用过一次伏立康唑的住院患者纳入研究,患者平均年龄为58岁,男性占65.4%,平均住院时间27.80 d,应用最多的为呼吸科和血液科。微生物样本送检率为95.3%,分离培养出病原菌61株,以白色念珠菌为主,样本多来源于痰。340例伏立康唑临床应用不合理,表现为适应证不适宜(19.06%)、药物选择不适宜(4.24%)、药物剂型或给药途径不适宜(12.24%)、用法用量不适宜(80.00%)、不良的药物相互作用(2.12%)。结论 临床伏立康唑不合理用药现象严重,临床医师在用药过程中应注意用法用量和药物相互作用等问题,促进伏立康唑的安全、合理使用。 |

| Objective To investigatethe clinical application of voriconazole for the prevention and treatment of invasive fungal diseases and analyze the rational useof voriconazole. Methods A retrospective statistical analysis was conducted of hospitalized patients using voriconazole between January 1,6 and December 31,6 in Department of Pharmacy,The First Affiliated Hospital of Anhui Medical University.The rational use of voriconazole was evaluated,according to the evaluation standards. Results A total of 425 patients wereincluded.The average age of the patients was 58 years old,male accounted for 65.4%,and the average length of stay was 27.80 d.Voriconazole was most widely used in department of respiration and department of hematology.The microbiological specimen submission rate was 95.3%.Sixty-one strains of pathogens were isolated and cultured.Candida albicans was the main source of samples,and the samples were mostly from sputum.Clinical application ofvoriconazole in 340 cases was not reasonable.The irrational rates of indication,drug selection,form or route of administration,dose and dosage and adverse drug interactions were 19.06%,4.24%,12.24%,80.00%,and 2.12%,respectively. Conclusions The irrational use of voriconazole is serious.Clinicians should pay attention to the usage and dosage and drug interaction problems in practice,to ensure medication safety and to promote rational use of voriconazole. |
