| 目的 探讨老年胆总管结石伴乳头旁憩室的临床特点及内镜治疗效果。 方法 回顾分析117例老年患者(＞60岁)的临床资料。 结果 117例患者,憩室组术前经磁共振胰胆管成像考虑有胆总管结石48例,术中经内镜逆行胰胆管造影未发现结石5例,误诊率10.42%；确诊的43例胆总管结石患者有39例(90.70%)均一次性成功取石。非憩室组术前经磁共振胰胆管成像考虑有胆总管结石69例,术中经内镜逆行胰胆管造影造影未发现结石1例,误诊率1.45%。确诊的68例胆总管结石有65例(95.59%)均一次性成功取石。 结论 对于老年胆总管结石伴乳头旁憩室患者临床更易误诊,内镜下诊疗是一种效果肯定、创伤小的方法,值得临床推广。 |

| Objective To investigate the clinical characteristics and endoscopic treatment of senile choledocholithiasis with papillary diverticula. Methods The clinical data of 117 elderly patients (＞60 years) were retrospectively analyzed. Results Among 117 patients,in the diverticulum group,there were 48 cases diagnosed with likely common bile duct stones after magnetic resonance cholangiopancreatography,yet endoscopic retrograde cholangiopancreatography showed no stones in 5 cases,the misdiagnosis rate being 10.42%.Of the 43 cases with confirmed choledocholithiasis,stones were successfully removed in 39 (90.70%) cases at the first time.In the non-diverticulum group,there were 69 cases diagnosed with likely common bile duct stones after magnetic resonance cholangiopancreatography,yet endoscopic retrograde cholangiopancreatography showed no stones in 1 case,the misdiagnosis rate being 1.45%.Of the 68 cases with confirmed choledocholithiasis,stones were successfully removed in 65 cases (95.59%) at the first time. Conclusions For elderly patients of choledocholithiasis with papillary diverticula,they are more likely to be misdiagnosed.Endoscopic diagnosis and treatment may achieve exact curative effect and small trauma,which is worthy of clinical promotion. |
