      目的 系统评价长春西汀联合甲钴胺对治疗糖尿病周围神经病变(DPN)患者的临床疗效,为临床提供循证参考。 方法计算机检索维普数据库、万方数据库、中国期刊全文数据库、中国生物医学文献数据库、Cochrane Library、PubMed等数据库(检索截止时间为2016年10月31日),收集有关长春西汀联合甲钴胺治疗对DPN患者疗效的随机对照研究(RCT)。纳入研究观察组采用长春西汀联合甲钴胺治疗,对照组单用甲钴胺治疗。由2名研究者按纳入与排除标准独立进行筛选,并对纳入研究的文献质量进行评价,提取数据资料;应用RevMan 5.3软件对数据进行Meta分析,绘制森林图和漏斗图。 结果 共纳入13篇符合标准的RCT,共1 274例DPN患者,其中观察组643例,对照组631例。Meta分析结果显示,观察组总有效率是对照组的1.35倍[RR=1.35,95% CI(1.26~1.43),P<0.000 01];观察组正中神经运动传导速度(MNCV)优于对照组[352例 vs 343例,MD=4.33,95% CI(1.64~7.02),P=0.002 ];观察组腓总神经MNCV优于对照组[375例 vs 363例,MD=3.60,95% CI(3.11~4.09),P<0.000 01];观察组正中神经感觉传导速度(SNCV)优于对照组[382例 vs 373例,MD=3.54,95% CI(2.54~4.54),P<0.000 01];观察组腓总神经SNCV优于对照组[299例 vs 288例,MD=3.78,95% CI(2.64~4.93),P<0.000 01],结果均差异有统计学意义(P<0.05)。 结论 长春西汀联合甲钴胺治疗糖尿病周围神经病变提高临床疗效,改善神经传导速度,但尚需更多高质量临床随机对照研究进一步证实。

      Objective Injection combined with Mecobalamine for treating diabetic peripheral neuropathy (DPN) and to provide evidence-based reference for clinic. Methods The randomized controlled trials(RCT) on Vinpocetine Injection combined with Mecobalamine treatment of diabetic peripheral neuropathy were retrieved from VIP,Wanfang Database,CNKI,CBM,the Cochrane library and PubMed (The end-date-time was October 31,6).Random number table was used to assign patients into experimental group and control group.The experimental group adopted vinpocetine injection combined with mecobalamine while the control group adopted mecobalamine.Two reviewers independently screened RCTs according to the inclusion and exclusion criteria,and assessed the methodological quality of the included trials for extracting the data.A meta-analysis of the obtained data was performed by the Rev Man 5.3 software.Forest plots and funnel plots were drawn. Results A total of 13 RCTs involving 1 274 patients were obtained,643 cases in experimental group and 631 cases in control group.Meta-analysis showed that the total effective rate of the experimental group was higher than that of the control group [RR=1.35,5% CI(1.26-1.43),P<0.000 01];the improvement of the median nerve′s motor nerve conduction velocity (MNCV) [MD=4.33,5% CI(1.64-7.02),P=0.002],the improvement of the peroneal nerve′s MNCV [MD=3.60,5% CI(3.11-4.09),P<0.000 01],the improvement of the median nerve′s sensory nerve conduction velocity (SNCV) [MD=3.54,5% CI(2.54-4.54),P<0.000 01] and the improvement of the peroneal nerve′s SNCV [MD=3.78,5% CI(2.64-4.93),P<0.000 01] in the experimental group were better than those of the control group.The difference was statistically significant (P<0.05). Conclusions The combination of vinpocetine injection and mecobalamine can improve clinical efficacy and improve nerve conduction velocity.Yet more high-quality randomized clinical trials are still needed to further validate the results.
