| 目的 研究内镜下密集套扎治疗重度食管静脉曲张的疗效。方法 对2013年11月至2016年12月在延安大学附属医院行内镜下套扎治疗的118例患者进行回顾性分析。按操作的方法将其分为密集套扎组和非密集套扎组。比较两组间疗效及并发症的发生情况。结果 第一次套扎治疗术后密集套扎组静脉曲张改善显效率60.32%,明显高于非密集套扎组41.82%(χ2=4.024 ,P<0.05)。密集套扎组套扎治疗术后静脉曲张消除率明显高于非密集套扎组[第1次套扎后19(30.50%)比8(14.54%)(χ2=4.057 ,P= 0.044)；第2次套扎后50(79.37%)比20(36.36%)(χ2=22.502 ,P=0.000)；第三次套扎后58(92.06%)比30(54.55%)(χ2=21.800,P=0.000)]。使静脉曲张完全消除的套扎次数密集套扎组明显少于非密集套扎组。而在急诊止血率、术后出血、穿孔、发热及胸骨后疼痛等并发症方面均差异无统计学意义。结论 密集套扎可以显著缩短食管静脉曲张消除的时间,减少内镜下治疗的次数,减轻患者痛苦。 |

| Objective This study is to investigate the therapeutic effects of intensive endoscopic variceal ligation in the treatment of severe esophageal varices.Methods This retrospective study included 118 patients who underwent endoscopic variceal ligation in our hospital from November 2013 to December 2016.Patients were divided into the intensive and non-intensive variceal ligation groups.Varix improvement rates and disease complications were analyzed and compared between these two groups.Results For the intensive ligation group,the varix improvement rate after the first ligation treatment was 60.32%,which was significantly higher than the non-intensive ligation group 41.82%,χ2=4.024,P<0.05.Moreover,the varicose vein-eliminating rates for the intensive ligation group were significantly higher than the non-intensive ligation group [after the first variceal ligation treatment,19 (30.50%) for the intensive vs.8 (14.54%) for the non-intensive ligation groups,χ2=4.057,P=0.044;after the second variceal ligation treatment,50 (79.37%) for the intensive vs.20 (36.36%) for the non-intensive ligation groups,χ2=22.502,P=0.000;and after the third variceal ligation treatment,58 (92.06%) for the intensive vs.30 (54.55%) for the non-intensive ligation groups,χ2=21.800,P=0.000].However,there were no significant differences in the emergency hemostasis,postoperative bleeding,perforation,or complications (fever and retrosternal pain) between these two groups.Conclusion Intensive endoscopic variceal ligation could significantly shorten the eliminating time of severe esophageal varices,decline the number of endoscopic treatment,and reduce the patients pain. |
