| 目的 评价直肠癌保肛术对肛门功能的影响,并研究综合治疗对肛门功能康复的促进作用。方法 选取2014年1月至2016年1月荆门市第二人民医院收治的104例直肠癌患者为研究对象,将患者以随机数字表法随机分成4组,每组各26例,4组患者术后均给予临床常规化疗,并于术后1个月时分别给予第1组生物反馈疗法,第2组 中医辨证疗法,第3组生物反馈联合中医辨证疗法,而第4组患者术后不行任何临床干预。采用Wexner便秘评分表评价4组患者的肛门功能,比较分析4组患者术前,术后1、3、6个月的肛管直肠测压指标。结果 直肠癌患者行保肛术后1个月时的肛门功能明显不如术前水平(P<0.05)；术后3个月时,第3组的肛门功能康复情况最佳(P<0.05)；术后6个月时,第1组和第2组的肛门功能康复情况接近术前水平,第3组肛门功能康复基本达到术前水平,第4组肛门功能康复情况仍不如术前水平(P<0.05)；第4组的两年复发率高于前三组。结论 直肠癌保肛术会对肛门功能造成影响,但通过术后综合治疗会使肛门功能得到康复,采用中医辨证疗法联合生物反馈疗法更利于直肠癌保肛术后患者肛门功能的康复。 |

| Objective To evaluate the effect of anus preserving operation on anal function and study the effect of comprehensive treatment on anal function.Methods 104 cases of patients with rectal cancer in our hospital from January 2014 to January 2016 were selected as the object of study.The patients were randomly divided into four groups,each 26 cases in each.The patients in four groups were given routine clinical chemotherapy,while the biological feedback therapy were taken in the first group at one month after operation,the second group treated with Chinese medicine dialectical therapy,the third group treated with biological feedback combined with TCM therapy.The patients in the four groups were not given any clinical intervention.The Wexner constipation score evaluation of anal function in patients in the four groups.The four groups of patients with preoperative and postoperative one month,after three months and after six months of anorectal manometry compared and analyzed.Results One month after anus preserving operation in patients with rectal cancer insurance of anal function was obviously less than that of preoperative levels (P<0.05).Three months after operation,the third group of anal function recovery was the best (P<0.05).Six months after the operation,the first group and the second group of anal function rehabilitation close to preoperative levels,and the third group of anal function recovered to the preoperative level, while the fourth group of anal function recovery is still not as good as the preoperative level (P<0.05).The recurrence rate of the fourth groups was higher than that of the first three groups.Conclusions Rectal cancer with preservation of the anus preserving operation will have an impact on the anal function,however,by postoperative comprehensive treatment will make anal functional rehabilitation.The Chinese medicine dialectical therapy combined with biological feedback therapy is benefit for rehabilitation of rectal cancer with preservation of the anus after operation of anal function. |
