| 目的 评估113例口腔恶性肿瘤患者的术后生存质量, 并且分析影响其生存质量的因素。方法 选取安徽医科大学第一附属医院口腔颌面外科113例口腔恶性肿瘤患者,采用华盛顿大学生存质量问卷(UW-QOL)对患者术后生存质量进行统计,并讨论影响生存质量的可能因素。结果 单因素分析提示:颌骨连续性、颈清方式、临床分期、肿瘤复发、放化疗等均会对患者术后生存质量产生不同程度的影响(P＜0.05)。多因素分析提示:肿瘤复发对患者生存质量影响最大(标准化系数:0.405)其次为颌骨连续性(标准化系数:-0.303),肿瘤分期(标准化系数:-0.226),有无颈清(标准化系数:0.216)。结论 影响口腔恶性肿瘤患者术后生存质量的相关因素,主要为肿瘤复发、 颌骨连续性、肿瘤分期以及颈清方式等。因此在以后的肿瘤治疗过程中应从以下几个方面入手,提高患者术后生存质量:首先提高肿瘤早期检出率,手术过程中尽量完整切除肿瘤,避免残留,必要时可行颈部淋巴结清扫。其次对颌骨不连续患者及时行缺损的重建与修复,尽量恢复患者颌面部形态与功能。 |

| Objective To assess the quality of life in oral cancer patients after operation,and to analyze the factors that affect the quality of life.Methods 113 oral cancer patients who had surgery in the department of stomatology at the First Affiliated Hospital of Anhui Medical University were selected.The quality of life of the patients was assessed by UW-QOL,and the factors influencing the quality of life were analyzed.Results The univariate analysis suggested that jaw continuity,neck dissection,clinical stage,tumor recurrence,radiotherapy and chemotherapy all had different effects on postoperative quality of life (P<0.05).Multivariate analysis suggested that tumor recurrence had the greatest impact on the patients quality of life (standardized coefficient:0.405) followed by jaw continuity (standardized coefficient:-0.303),tumor stage (standardized coefficient:-0.226),presence or absence of neck clearing (normalized coefficient):0.216).Conclusion The related factors affecting the quality of life of patients with oral cancer are tumor recurrence,discontinuous jaws,clinical stage and neck dissection.Therefore,we can improve the quality of life in the following aspects:first,we should improve the detection rate of the early tumor,and we should completely remove the tumor to avoid residual during the operation;secondly,the patients with discontinuous jaws should be reconstructed and repaired in time,so as to reconstruct the facial morphology and function of the patients as much as possible. |
