| 目的 探讨阴道三维能量多普勒超声(3D-CPA)、核磁共振扩散加权成像(DWI)对子宫内膜良恶性疾病的诊断价值。方法 收集2015年5月至2017年5月青海大学附属医院、青海省人民医院绝经后阴道不规则流血的90例患者,分别行3D-CPA、DWI检查,同时行宫腔镜、诊刮手术获取病理组织,分析两种影像学检查与病理检查的一致性,分析3D-CPA、DWI诊断子宫内膜疾病的价值。结果 (1)3D-CPA与DWI对子宫内膜良恶性疾病的诊断价值:3D-CPA对子宫内膜疾病诊断符合率91.1%(82/90)与 DWI对子宫内膜疾病诊断符合率93.3%(84/90)相比,差异无统计学意义(P＞0.05),提示3D-CPA诊断子宫内膜疾病有较高的准确性；与病检结果相比,3D-CPA的敏感性和特异性分别为85.54%、50.00%,DWI的敏感性和特异性分别为85.36%、52.17%。(2)Kappa检验结果显示,3D-CPA和病检(Kappa=0.704)、DWI和病检(Kappa=0.800)、3D-CPA和DWI(Kappa=0.877)具有较高一致性。结论 DWI和3D-CPA在诊断子宫内膜疾病方面,均为较好的诊断手段,3D-CPA更适于对“高危人群”进行连续追踪随访。 |

| Objective To investigate the diagnostic value of trans-vaginal three-dimensional energy Doppler ultrasonography (3D-CPA) and magnetic resonance diffusion weighted imaging (DWI) in benign and malignant endometrial diseases.Method The treatment data from 90 patients with the symptom of postmenopausal vaginal irregular bleeding admitted in the Peoples Hospital of Qinghai Province and the Affiliated Hospital of Qinghai University May of 2015 up to 2017 were taken and then dissected to obtain pathologic tissue through 3D-CPA,DWI as well as hysteroscopic curettage surgery in an effort to analyze the consistency between the pathological examination and medical imaging examination and the diagnostic value for the endometrial diseases by means of 3D-CPA and DWI. Results (1)The value of 3D-CPA and DWI in the diagnosis of endometrial benign and malignant diseases: the coincidence rate of 3D-CPA 91.1% in the diagnosis of endometrial disease (82/90) and DWI in line with the rate of diagnosis of 93.3% endometrial disease (84/90) (P＞0.05),suggesting that 3D-CPA in the diagnosis of endometrial diseases with high accuracy.Compared with the pathological results,the sensitivity of 3D-CPA and the specificity was 85.54% and 50.00%,the sensitivity and specificity of DWI were 85.37% and 52.17%.(2)Kappa test showed that 3D-CPA and pathological results (Kappa=0.704),DWI and pathological results (Kappa=0.800),3D-CPA and DWI (Kappa=0.877) had higher consistency.Conclusion DWI and 3D-CPA are better diagnostic methods in the diagnosis of endometrial diseases and 3D-CPA is more suitable for continuous follow-ups of high-risk groups. |
