      目的 比较口服两种不同剂量维生素D3对早产超低和极低出生体质量儿骨代谢指标的影响。 方法 选择早产超低和极低出生体质量儿60例,出生后10 d起在喂养耐受情况下口服补充维生素D3,用随机数字表法分为高剂量组(800 IU·d-1)30例和低剂量组(400 IU·d-1)30例,在出生后28 d比较两组患儿骨代谢指标血清钙(Ca)、磷(P)、碱性磷酸酶(AKP)水平,出院前比较早产儿代谢性骨病等并发症发生率。 结果 两组患儿在出生时血清Ca、P、AKP值比较均差异无统计学意义(P>0.05),分别口服补充维生素D3后,在出生后28 d两组患儿血清Ca、AKP值呈升高趋势,P值呈降低趋势;低剂量组血清P值明显低于高剂量组[(1.43±0.34)mmol·L-1 比(2.00±0.34)mmol·L-1],AKP值明显高于高剂量组[(468.93±98.97)U·L-1比(385.00±116.28)U·L-1],均差异有统计学意义(t=6.60、-3.01,均P<0.01),两组血清Ca值比较差异无统计学意义(P>0.05),两组早产儿代谢性骨病等并发症发生率比较差异无统计学意义(P>0.05)。 结论 高剂量口服补充维生素D3 800 IU·d-1可改善早产超低和极低出生体质量儿骨代谢指标异常。

      Objective To compare the effects of two different doses of vitamin D3(VD3)on the bone metabolic markers in extremely and very low birth weight infants. Methods A total of 60 extremely and very low birth weight infants were enrolled in the study.VD3 supplement was given from the 10 days after birth when they could be orally fed.The infants were randomized into high dose group (800 IU·d-1) and low dose group (400 IU·d-1)with 30 infants in each group.Serum calcium (Ca),phosphorus (P) and alkaline phosphatase (AKP) levels were detected and compared between groups 28 days after birth.The morbidity of complications such as metabolic bone disease of prematurity (MBDP) was compared at discharge. Results There were no significant differences between the two groups in the levels of serum Ca,P and AKP at birth.After being given VD3 at two different doses respectively,serum Ca,AKP levels showed an upward trend at 28 days after birth,while Plevel showed downward.The level of serum Pin low dose group was lower than that in high dose group [(1.43±0.34)mmol·L-1 vs.(2.00±0.34)mmol·L-1],while the level of AKP in low dose group was higher [(468.93±98.97)U·L-1 vs.(385.00±116.28)U·L-1];the differences were statistically significant(t=6.60,-3.01,P<0.01).There were no significant differences between the two groups in the level of Ca (P>0.05) and the morbidity of complications(P>0.05). Conclusions High dose of VD3 supplement(800 IU·d-1) can improve the abnormal bone metabolism of extremely and very low birth weight infants.
