| 目的 对血清糖蛋白-2α(GP-2α)在重症急性胰腺炎(SAP)中的诊断及预后中的评估价值进行研究。方法 选择2012年6月至2016年6月上海市浦东新区周浦医院48例SAP患者,在入院第1、3、5、7、14天检测外周静脉血中GP-2α水平,并与同期40例轻症急性胰腺炎(MAP)患者作对照研究。结果 SAP患者的血清GP-2α水平在入院后第3天[(22.70±5.92) U vs.(12.91±3.78) U]、第5天[(25.61±11.23) U vs.(13.25±7.94) U]、第7天[(15.60±6.22) U vs.(6.69±5.53) U]、第14天[(14.37±6.79) U vs.(3.41±1.08) U]均显著高于MAP患者；治愈的SAP组患者入院后血清GP-2α水平逐渐升高,于第5天[(25.61±11.23) U]达到峰值,随后逐渐下降,在第14天[(14.37±6.79) U]仍显著高于MAP组患者；死亡组的SAP患者GP-2α升高后不再降低。第3天的GP-2α水平[(38.44±16.98)U]对于辨别SAP和MAP价值高于糖蛋白-2水平。结论 GP-2α对于重症急性胰腺炎的早期诊断及预后特异性高,持续时间较长,可能具有潜在的临床应用价值。 |

| Objective To explore the value of serum glycoprotein 2α (GP-2α) in the diagnosis and prognosis of severe acute pancreatitis (SAP).Methods The levels of GP-2α in peripheral intravenous serum were detected at 1,3,5,7 and 14 days of hospitalization in 48 patients with SAP from June 2012 to June 2016 in Zhoupu Hospital of Shanghai,and the data were compared with those of 40 mild acute pancreatitis (MAP) patients.Results Serum GP-2α levels in SAP patients were significantly higher than MAP patients at 3 days [(22.70±5.92) U vs.(12.91±3.78) U],5 days [(25.61±11.23) U vs.(13.25±7.94) U],7 days [(15.60±6.22) U vs.(6.69±5.53) U],14 days [(14.37±6.79) U vs.(3.41±1.08) U] after admission.For recovered SAP patients,serum GP-2α level increased gradually after admission in the SAP group,peaked on the 5th day [(25.61±11.23) U],and then gradually decreased.On the 14th day,GP-2α [(14.37±6.79) U] was still significantly higher than in the MAP group,and the SAP patients in the death group had GP-2α elevated and no longer lowered.The predicted value of serum GP-2α level [(38.44±16.98) U] on the third day for distinguishing between SAP and MAP was higher than the GP-2 level.Conclusion GP-2α is highly specific for the early diagnosis and prognosis of SAP,and has a long duration,which may have potential clinical value. |
