      目的 探讨个体化饮食指导对慢性肾衰竭(CRF)维持性血液透析(MHD)患者营养状况及血磷、血糖管理的影响。方法选取大连医科大学附属第一医院2015年1月至2016年1月未接受个体化饮食指导的MHD治疗CRF患者49例为对照组,2016年2月至2017年1月接受个体化饮食指导的MHD治疗CRF患者49例为观察组。比较两组干预前后血钙、血磷、血清白蛋白(ALB)、前白蛋白(PA)、转铁蛋白(SF)、体质量指数(BMI),干预前后评价主观综合性营养评估(SGA)、肾病饮食依从态度量表(RAAQ)与行为量表(RABQ)评分,并统计低血糖发生率。结果 干预后,两组的ALB、PA、SF、BMI、血钙、RAAQ、RABQ评分均升高,血磷和SGA评分均降低,且观察组的改善幅度显著优于对照组,差异有统计学意义(P<0.05);观察组低血糖发生率显著低于对照组(4.08% vs.18.37%),差异有统计学意义(P<0.05)。结论 个体化饮食指导提高MHD治疗CRF患者的饮食依从性,可改善其营养状况及钙磷代谢,降低低血糖发生率并改善患者的生存质量。

      Objective To investigate the effects of individual diet guidance on nutritional status,calcium-phosphorus metabolism and blood glucose management in maintenance hemodialysis (MHD) patients with chronic renal failure (CRF).Methods Forty-nine patients with CRF treated by MHD without individual diet guidance in The First Affiliated Hospital of Dalian Medical University from January 2015 to January 2016 were enrolled as control group,while 49 patients with CRF treated by MHD with individual diet guidance from February 2016 to January 2017 were selected as observation group.The blood calcium,serum phosphorus,serum albumin (ALB),prealbumin (PA),transferrin (SF),body mass index (BMI) were compared before and after the intervention;the subjective global assessment (SGA),renal adherence attitudes questionnaire (RAAQ) and renal adherence behavior questionnaire (RABQ) scores were evaluated,and the incidence of hypoglycemia were counted.Results After intervention,the levels of ALB,PA,SF,BMI,blood calcium and scores of RAAQ,RABQ in the two groups were significantly increased,while the blood phosphorus and SGA scores were significantly decreased,and the changes in the observation group were significantly better than those in the control group,and the differences were statistically significant (P<0.05).The incidence of hypoglycemia in the observation group was significantly lower than that of the control group,and the difference was statistically significant [(4.08% vs.18.37%),P<0.05].Conclusion Individual diet guidance can improve the dietary compliance of CRF patients treated with MHD,improve nutritional status and calcium-phosphorus metabolism,reduce the incidence of hypoglycemia and improve the quality of life of patients.
