      目的观察支原体肺炎病儿体内 Toll样受体 4(TLR4)、 CXCL8、可溶性髓系细胞触发受体 ?1(sTREM?1)水平,并分析其临床意义。方法选取 2016年 12月至 2017年 10月在郑州大学附属儿童医院接受治疗的支原体肺炎病儿 100例为观察组,并按不同疾病程度再分为重症组 35例和非重症组 65例;选取同期接受体检的健康儿童 100例为对照组。观察两组及不同疾病程度组 TLR4、CXCL8、sTREM?1水平、炎症因子和肺功能指标的差异,分析各指标与炎症因子和肺功能的相关性。结果支原体肺炎病儿的 TLR4[(3.48±0.67)vs.(1.03±0.02)ng/mL]、 CXCL8[(288.67±12.65)vs.(148.53±19.12)pg/mL]、 sTREM?1[(55.36 ±5.69)vs.(18.42±3.12)pg/mL]、 TNF?α[(24.55±4.18)vs.(9.06±1.65)pg/mL]、 IL?6[(45.25±9.58)vs.(9.06±1.65)pg/mL]、 hs?CRP[(5.63±1.35)vs.(1.68±0.72)mg/L]水平均高于对照组( P<0.05)FVC、MMEF、PEF水平低于对照组( P<0.05);重症支原体肺炎病儿的 TLR4、CXCL8、sTREM?1、TNF?α、IL?6、hs?CRP水平高于非,重症组( P<0.05),FVC、MMEF、PEF水平低于非重症组( P<0.05);支原体肺炎病儿的 TLR4、CXCL8、sTREM?1水平与炎症因子水平正相关,与肺功能水平负相关。结论支原体肺炎病儿的 TLR4、CXCL8、sTREM?1水平较高,与炎症因子和肺功能指标相关,且受疾病严重程度的影响。

      Objective To observe the level of TLR4,CXCL8,sTREM?1 in children with Mycoplasma pneumoniae and to analyze its clinical significance.Methods The children of Mycoplasma pneumoniae treated in our hospital from December 2016 to Octobe2017 were selected as the research subjects,and the healthy children who received physical examination in our hospital for thesame period were selected as the control.The differences of TLR4,CXCL8,sTREM?1,inflammatory factors and lung function were observed between the two groups,and the correlation of TLR4,CXCL8,sTREM?1 levels with inflammatory factors and lung function in children with mycoplasma pneumonia was analyzed.Results The levels of TLR4[(3.48±0.67)vs.(1.03±0.02)ng/mL],CXCL8[( 288.67±12.65)vs(148.53±19.12)pg/mL]sTREM?1[( 55.36±5.69)vs.(18.42±3.12)pg/mL],TNF?α[( 24.55±4.18)vs.(9.06±1.65)pg/mL],IL?6[(45.25±9.58)vs.(96±1.65)pg/mL]and hs?CRP[(5.63±1.35)vs.(1.68±0.72)mg/L]in children with Mycoplasma pneumoniae were higher than those in the control group(P<0.05).The levels of FVC,MMEF and PEF were low?er than those of the control group(P<0.05).The levels of TLR4,CXCL8,sTREM?1,TNF? a,IL?6 and hs?CRP in children with se? vere mycoplasma pneumonia were higher than those in non severe group(P<0.05).The levels of FVC,MMEF and PEF were lower than those of the control group(P<0.05).Conclusion The levels of TLR4,CXCL8,sTREM?1 in children with mycoplasma pneu?monia are relatively high,which are related to inflammatory factors and lung function indexes,and are affected by the severity of .0,the disease.
