      目的观察康柏西普联合 577 nm激光治疗糖尿病性黄斑水肿( DME)的疗效。方法采用回顾性研究,选取遵义医科大学第三附属医院 2016年 3月至 2019年 4月收治的 DME病人 96例( 99只),根据治疗方式的不同将其分为对照组( n=48例, 49只)和观察组( n=48例, 50只)。对照组予以 577 nm激光光凝治疗,在此基础上,观察组加用玻璃体内注射康柏西普治疗。比较两组临床疗效、并发症情况,检测两组不同时间点的黄斑区血流密度、血管新生因子表达量、最佳矫正视力、黄斑中心厚度以及黄斑区荧光渗漏情况的差异。结果治疗后 4个月,观察组与对照组比较,浅层视网膜黄斑区血流密度[(52.23±3.41)比(48.59 ± 2.75)]、深层视网膜黄斑区血流密度[( 50.38 ± 3.09)比( 46.50 ± 2.62)]以及血管内皮生长因子 165b(VEGF165b)[(157.28±12.11)ng/L比( 395.73±32.38)ng/L]、内皮素 ?1(ET?1)[(93.88±8.62)ng/L比( 144.19±11.63)ng/L]、基质细胞衍生因子 SDF?1(SDF?1)[(0.58±0.19)ng/mL比( 1.01±0.26)ng/mL]、 β2?糖蛋白 Ⅰ(β2?GPⅠ)[(0.72±0.11)ng/L比( 0.96±0.18)ng/L]等血管新生因子水平,观察组改善程度更为明显,差异有统计学意义( P<0.05);治疗后 1、2个月,观察组视力、黄斑中心厚度显著优于对照组,差异有统计学意义( P<0.05);治疗后 2、4个月观察组有渗漏眼数( 10、9只)显著低于对照组( 19、19只)差异有统计学意义( P<0.05);此外,随访 2个月期间,观察组眼压升高发生例数( 9只)明显低于对照组( 17只),差异有统计学,意义( χ2= 4.38,P=0.04)。结论玻璃体内注射康柏西普联合 577 nm激光治疗 DME的疗效确切,安全性较高,同时可明显减少病人视网膜血管活性介质的释放,起到提高视功能、减轻黄斑区水肿和渗漏以及改善浅层、深层视网膜黄斑区血流密度等作用。

      Objective To investigate the therapeutic effect of compassip combined with 577 nm laser on diabetic macular edema(DME).Methods A retrospective study found that 96 patients(99 eyes)with DME admitted to the Third Affiliated Hospital ofZunyi Medical University from March 2016 to April 2019 were divided into control group(n=48 cases,49 eyes)and research group(n=48 cases,50 eyes)according to different treatment methods.The control group was treated with 577 nm laser photocoagu? lation.On this basis,the study group was treated with intravitreal injection of Compoxip.Compared the clinical efficacy and complica?tions of the two groups.Detected the differences of blood flow density,expression of angiogenesis factor,best corrected visual acuity, central macular thickness and fluorescence leakage in the macular area between the two groups at different time points.
