| 目的探讨硝呋太尔制霉菌素阴道软胶囊联合奥硝唑用于治疗滴虫性阴道炎的临床疗效及其对辅助性 T细胞( Th1/ Th2)因子水平及微循环的影响。方法选择 2016年 2月至 2017年 10月于南京医科大学附属无锡妇幼保健院妇产科进行治疗并且确诊为滴虫性阴道炎的病人 130例，按照随机数字表法分为研究组 65例和对照组 65例，研究组给予硝呋太尔制霉菌素阴道软胶囊联合奥硝唑进行治疗，对照组给予奥硝唑进行治疗。观察病人的治疗有效率、治疗前后 Th1和 Th2细胞因子水平和微循环指标的变化以及疾病复发率。结果研究组治疗有效率为 93.85%显著高于对照组 76.92%(Z＝9.621，P＝0.004)研究组 3个月和 6个月内的复发率为 6.15%和 16.92%显著低于对照组病人复发率 26.15%和 38.46%，且差异有统计学意义(3个，月比较， χ2＝9.600，P＝0.002；6个月比较， χ2＝7.530，P＝0.006)。治疗前，两组病人的白细胞介素( IL)?2、IL?4、IL?10及干扰素 γ(IFN?γ)水平差异无统计学意义( P＞0.05)；治疗后，两组病人的 IL?2、IL?4、IL?10及 IFN?γ水平均较治疗前显著降低，且研究组水平显著低于对照组( P＜0.05)；另外，治疗后，两组病人的 IL?2/IL?4、IL?2/IL?10、IFN?γ/IL?4和 IFN?γ/IL?10比值较治疗前升高，且研究组显著高于对照组( P＜0.05)；治疗后，两组病人微循环指标均得到改善，研究组病人的微循环得到显著改善且优于对照组( P＜0.05)。结论硝呋太尔制霉菌素阴道软胶囊联合奥硝唑能够有效治疗滴虫性阴道炎，能够有效维持机体免疫功能的平衡，改善微循环，显著降低复发率。 |

| Objective To explore the effect of nifuratel nystatin vaginal soft capsules combined with ornidazole in treating tricho? monas vaginitis，and the effect on the levels of Th1 and Th2 cell factors and microcirculation.Method A total of 130 patients withtrichomonal vaginitis admitted to Wuxi Maternal and Child Health Hospital Affiliated to Nanjing Medical University from Feb 2016to October 2017 were selected and randomly divided into the control group and the study group，with 65 cases in each group.The study group was treated with nifuratel nystatin vaginal soft capsules combined with ornidazole，while the control group was treated with ornidazole.The clinical efficacy，the disease relapse，the levels of Th1 and Th2 cell factors，and the microcirculation index were observed. Results The effective rate in the study group was 93.85%，which was significant higher than that in the control group76.92%(Z＝9.621，P＝0.004).The recurrence rates of the study group within 3 and 6 months were 6.15% and 16.92%，re? spectively，which were significantly lower than those of the control group(26.15% and 38.46%，respectively)，and the difference was statistically significant(χ2＝9.600，P＝0.002；χ2＝7.530，P＝0.006).Before treatment，there were no difference between the two groups in the levels of IL?2.IL?4，IL?10 and INF?γ(P＞0.05)； After treatment，the levels of IL?2，IL?4，IL?10 and INF?γ in both groups were significant decreased(P＜0.05)，and the above factors of the study group were significantly lower than those of the control group(P＜0.05).After treatment，the rates of IL?2/IL?4，IL?2/IL?10，IFN?γ/IL?4 and IFN?γ/IL?10 of the two groups were in? creased，and the above rates of the study group were significantly higher than those in the control group(P＜0.05).After the treat? ment，the microcirculation indexes of the two groups were improved，and that of the patients in the study group was superior to that of the control group(P＜0.05).Conclusion Nifuratel nystatin vaginal soft capsules combined with ornidazole can effectively treat trichomonas vaginitis，adjust the immunity homeostasis balance，improve the microcirculation index，significantly decrease the dis?ease relapse. |
