| 目的探讨强化负压封闭引流(VSD)方案辅助高压氧疗对老年糖尿病足溃疡病人近期疗效及复发率的影响。方法选取 2016年 1月至 2018年 6月随州市中心医院收治老年糖尿病足溃疡病人 86例，以抽签法分为对照组(43例)和观察组(43例)，对照组在高压氧疗基础上加用常规 VSD辅助治疗，观察组在高压氧疗基础上加用强化 VSD方案辅助治疗；比较两组近期疗效，治疗前后足部症状评分、溃疡直径、密西根糖尿病周围神经病评分，足背动脉血流速度、踝肱指数、神经传导速度及随访复发率。结果对照组治愈 14例，有效 18例，无效 11例，观察组治愈 21例，有效 19例，无效 3例，观察组近期疗效显著优于对照组(P＜0.05)；观察组治疗后足部症状评分、溃疡长径、溃疡短径、密西根糖尿病周围神经病评分、足背动脉血流速度及踝肱指数均显著优于对照组、治疗前(P＜0.05)；随访 6个月，对照组复发率 11.62%(5/43)观察组复发率 0.00%(0/43)；观察组随访6个月复发率显著低于对照组(χ2＝6.739，P＝0.009)。结论强化 VSD方案辅助高压氧，疗治疗老年糖尿病足溃疡可有效改善临床症状，促进创面修复，增加足部微循环血流灌注，加快神经传导，且在降低远期复发率方面具有优势。 |

| Objective To explore the influence of intensive vacuum sealing drainage(VSD)regimen combined with hyperbaric ox?ygen therapy on short?term clinical efficacy and recurrence rate of elderly patients with diabetic foot ulcer.Methods Eighty?six el?derly patients with diabetic foot ulcer treated in Suizhou Central Hospital from January 2016 to June 2018 were chosen and random?ly assigned into 2 groups，including control group(43 patients)and observation group(43 patients).The control group was treatedwith conventional VSD regimen on the basis of hyperbaric oxygen therapy while the observation group was treated with intensiveVSD regimen on the basis of hyperbaric oxygen therapy.A comparison was made of the short?term clinical efficacy，foot symptom scores，ulcer diameters，Michigan diabetic peripheral neuropathy scores，dorsal pedal artery blood flow velocities，ankle?brachial in? dexes，nerve conduction velocities before and after treatment and recurrence rate during follow?up period between the two groups.Results In the control group 14 cases were cured，18 cases effective，and 11 cases ineffective while in the observation group 21 cases were cured，19 cases effective and 3 cases ineffective.The short?term clinical efficacy of observation group was significantlybetter than control group(P＜0.05).The foot symptom score，ulcer diameter，Michigan diabetic peripheral neuropathy score，dorsal pedal artery blood flow velocity，and ankle?brachial index of the observation group after treatment were significantly better thanthose of the control group and before treatment(P＜0.05).The recurrence rates during 6?month follow?up were 11.62%(5/43)in the control group and 0.00%(0/43)in the observation group(χ2＝6.739，P＝0.009).Conclusion Intensive VSD regimen com?bined with hyperbaric oxygen therapy in the treatment of elderly patients with diabetic foot ulcer can effectively improve clinicalsymptoms，promote wound repair，increase foot microcirculation blood perfusion volume，accelerate nerve conduction process and possess the advantages in reducing long?term recurrence rate. |
