      目的探究儿童闭塞性细支气管炎( BO)支气管肺泡灌洗液基质金属蛋白酶 9(MMP-9)、抗菌肽 37、中性粒细胞与气道炎症程度的关系。方法选取河南省儿童医院 2018年 1月至 2019年 6月 BO病儿 42例作为观察组,另选择该院同期支气管异物无并发感染病儿 38例作为对照组。均采用纤支镜进行肺泡灌洗,对比不同病情程度病儿支气管肺泡灌洗液 MMP-9、抗菌肽 37、中性粒细胞含量、气道炎症因子[白细胞介素 -17(IL-17)、白细胞介素 -13(IL-13)、半胱氨酰白三烯( LTS)]水平,分析 MMP9、抗菌肽 37、中性粒细胞与儿童 BO及气道炎症因子的影响关系或相关性。结果观察组支气管肺泡灌洗液 MMP-9、中性粒细胞含量分别为( 2 701.93±587.08)μg/L、(3.38±0.43)×109/L,高于对照组的( 135.48±44.10)μg/L、(2.76±0.25)×109/L,抗菌肽 37含量为( 114.28±24.98)μg/L,低于对照组的( 186.06±44.11)μg/L(P<0.05);支气管肺泡灌洗液 MMP-9、抗菌肽 37、中性粒细胞与儿童 BO有密切的影响或关联影响关系( P<0.05);随病情加重,支气管肺泡灌洗液 MMP-9、中性粒细胞、 IL-17、IL-13、LTS含量呈升高趋势,抗菌肽 37呈降低趋势( P<0.05); MMP-9、中性粒细胞与 IL-17、IL-13、LTS呈正相关,抗菌肽 37与 IL-17、IL-13、LTS呈负相关( P<0.05)。结论 BO病儿存在呼吸道 MMP-9、抗菌肽 37、中性粒细胞表达异常,其表达异常程度升高可能会加重气道炎症程度,可为临床诊断、制定治疗方案提供新方向。

      Objective To investigate the relationship between bronchoalveolar lavage fluid matrix metalloproteinase 9 (MMP-9), an? tibacterial peptide 37 (LL-37), neutrophil and the degree of airway inflammation in children with bronchiolitis obliterans (BO). Meth? ods Forty-two children with BO in our hospital from January 2018 to June 2019 were selected as the observation group,another 38children without concurrent infection of bronchial foreign bodies in the same period were selected as the control group. Alveolar lavagewas performed using fiberoptic bronchoscopy, the levels of bronchoalveolar lavage fluid MMP-9, LL-37, neutrophil content, airway in? flammatory factors [interleukin-17 (IL-17), interleukin-13(IL-13), cysteinyl leukotrienes (LTS)] in children with different levels of dis? ease were compared,the correlation between MMP-9, LL-37, neutrophils and airway inflammatory factors was analyzed. Results The contents of MMP-9 and neutrophils in the bronchoalveolar lavage fluid of the observation group were higher than those of the controlgroup[(2 701.93±587.08)μg/L vs. (135.48±44.10)μg/L,(3.38±0.43)×109/L vs. (2.76±0.25)×109/L], and the contents of LL-37 were lower than those of the control group [(114.28±24.98)μg/L vs. (186.06±44.11)μg/L](P<0.05); bronchoalveolar lavage fluid MMP-9, LL-37, and neutrophils were significantly correlated with children′s BO (P<0.05); with the worsening of the disease, the contents of MMP-9, neutrophils, IL-17, IL-13, and LTS in bronchoalveolar lavage fluid increased, and LL-37 decreased (P<0.05); MMP-9 and neutrophils were positively correlated with IL-17, IL-13, and LTS, and LL-37 was negatively correlated with IL-17, IL-13, and LTS (P<0.05). Con? clusion There is abnormal expression of MMP-9, LL-37 and neutrophils in children with BO, the abnormal expression may increasethe degree of airway inflammation, which can provide a new direction for clinical diagnosis and treatment plan.
