| 目的探讨微小 RNA-503(miRNA-503)在结肠癌病人血清中的表达及其临床意义。方法纳入 2012年 3月至 2013年 5月四川大学华西医院的 72例结肠癌病人和 50例健康体检者分别作为观察组和对照组，实时荧光定量聚合酶链式反应( qRTPCR)检测血清中 miRNA-503水平，分析其表达变化与临床病理特征的关系，受试者工作特征曲线( ROC曲线)评价其诊断价值。根据血清中 miRNA-503水平，将观察组病人分为高表达组和低表达组， Kaplan-Meire法比较两组病人的生存率。结果观察组血清中 miRNA-503水平( 0.49±0.02)显著低于对照组( 0.77±0.03)(P<0.05)。结肠癌病人血清中 miRNA-503水平与年龄、性别、肿瘤部位、分化程度、浸润深度、远处转移无关( P>0.05)而与肿瘤大小、临床分期、淋巴结转移有关( P<0.05)。 ROC曲线下面积为 0.859(P<0.05，95%CI:0.790~0.928)以 miRNA-503表，达量为 0.685作为截断值时，对结肠癌的诊断灵敏度为 0.875，特异度为 0.740。低表达组病人的生存率显于高表达组(P<0.05)。结论 miRNA-503在结肠癌病人血清中低表达，著低，与肿瘤大小、临床分期、淋巴结转移及预后不良有关，具有较好的诊断价值，可能成为潜在的诊断、治疗与预后靶点。 |

| Objective To explore the expression of microRNA503 (miRNA-503) in serum of patients with colon cancer and its clini? cal significance.Methods 72 patients with colon cancer and 50 healthy persons who were treated in Sichuan West China Hospitalfrom March 2012 to May 2013 were enrolled as observation group and control group respectively. The serum level of miRNA-503 was detected by real-time fluorescence quantitative polymerase chain reaction (qRT-PCR). The relationship between miRNA-503 expres?sion and clinicopathological characteristics was analyzed. The diagnostic value of microRNA503 was evaluated by receiver operatingcharacteristic curve (ROC curve).According to the serum level of miRNA-503, the patients in the observation group were divided into high-expression group and low-expression group. The survival rates of two groups were compared by Kaplan-Meire method.Results The serum level of miRNA-503 in the observation group (0.49±0.02) was significantly lower than that in the control group (0.77±0.03) (P<0.05).The serum level of miRNA-503 in patients with colon cancer was not related to age, sex, tumor location, degree of differentia?tion, depth of invasion and distant metastasis (P>0.05), but related to tumor size, clinical stage and lymph node metastasis (P<0.05).The area under ROC curve was 0.859 (P<0.05, 95%CI: 0.790-0.928). When 0.685 was treated as truncation value, the sensitivity and speci?ficity for the diagnosis of colon cancer were 0.875 and 0.740, respectively. The survival rate of low expression group was significantlylower than that of high expression group (P<0.05).Conclusion The low expression of miRNA-503 in the serum of patients with coloncancer is related to the size of tumors, clinical stage, lymph node metastasis and poor prognosis. It has good diagnostic value and maybecome a potential target for diagnosis, treatment and prognosis. |
