      目的本研究旨在探讨慢性心力衰竭病人血浆脂蛋白相关磷脂酶 A2(Lp-PLA2)水平与心功能分级的相关性。方法共收集了 2016年 1月至 2018年 10月在粤北人民医院心血管科住院的慢性心力衰竭病人 260例,其中 NYHA(纽约心脏病协会心功能分级) Ⅱ组 66例、 NYHA Ⅲ组 96例和 NYHA Ⅳ组 98例;收集心功能正常(92例)的病人作为对照组。超声心动图测定左室射血分数。采用免疫增强比浊法测定 Lp-PLA2水平。结果慢性心力衰竭病人血浆 Lp-PLA2水平明显高于对照组(P<0.05)。 NYHA Ⅳ组 Lp-PLA2水平高于 NYHA Ⅲ组, 402.50(356.00,435.00)比 322.00(223.00,407.50)(P<0.05)NYHA Ⅲ组 Lp-PLA2水平高于 NYHA Ⅱ组, 322.00(223.00,407.50)比 185.00(130.00,250.00)(P<0.05)。射血分数降低的心力衰,竭病人血浆 Lp-PLA2水平明显高于射血分数保留的心力衰竭病人(P<0.05)。结论我们的研究结果表明血浆 Lp-PLA2水平升高与慢性心力衰竭病人心功能分级的严重程度呈正相关。因此, Lp-PLA2也可能是慢性心力衰竭的新型炎症性生物标志物。

      Objective To assess the relationship of plasma lipoprotein-associated phospholipase A2 (Lp-PLA2) level and severity of cardiac function in chronic heart failure patients.Methods Two hundred and sixty patients admitted for the first time with chronicheart failure in Department of Cardiovascular, Yue Bei People's Hospital were collected and assigned into NYHA (the cardiac functionclassification standard of New York Heart Association) Ⅱ, NYHA Ⅲ and NYHA Ⅳ group. 92 patients with normal cardiac functionwere collected as control group. Ejection fraction (EF) was estimated by echocardiography. Lp-PLA2 level was measured by latex enhanced turbidimetric immunoassay.Results Plasma Lp-PLA2 level in patients with chronic heart failure was significantly elevated compared with control. Lp-PLA2 level was higher in NYHA Ⅳ group than in NYHA Ⅲ group [402.50(356.00,435.00) vs. 322.00(223.00, 407.50), P<0.05], and Lp-PLA2 level in NYHA Ⅲ group was higher than in NYHA Ⅱ group [322.00(223.00,407.50) vs. 185.00 (130.00, 250.00),P<0.05]. The plasma Lp-PLA2 levels in patients of heart failure with reduced EF (346.84±111.05) μg/L were significantly higher compared to that in patients of heart failure with preserved EF (284.57±113.30) μg/L (P<0.05).Conclusions Our findings demonstrated that the elevated plasma Lp-PLA2 level was associated with the severity of cardiac function in patients with chronic heart failure. Thus, Lp-PLA2 may also be an inflammatory biomarker of chronic heart failure.
