| 目的探讨加味苓桂术甘汤合针灸治疗卒中相关肺炎临床疗效，以及对血清炎症因子的影响。方法选取 2018年 1月至 2020年 3月安徽中医药大学第二附属医院确诊为卒中相关性肺炎病人 102例，采用随机数字表法分成 A、B、C三组， A组采用常规西药治疗； B组在 A组基础上口服加味苓桂术甘汤治疗； C组在 B组基础上联合针灸治疗。统计 3组治疗后临床总有效率、治疗前后血清炎症水平、神经功能损伤恢复情况及不良反应。结果治疗后， C组临床总有效率( 88.24%)明显高于 A组(70.59%)和 B组( 76.47%)( P< 0.05)；与治疗前比较，治疗后 3组的 C反应蛋白( C-reactive protein，CRP)、白细胞计数( white‐cell count，WBC)、降钙素原( procalcitonin，PCT)、卒中量表( NIH Stroke Scale，NIHSS)评分均显著降低( P< 0.01)；治疗后组间比较， C组的 WBC和 NHISS明显低于 A和 B组( P < 0.01)，CRP、PCT也低于 A组和 B组，差异有统计学意义( P < 0.05)。结论加味苓桂术甘汤合针灸治疗卒中相关性肺炎疗效好，能有效减轻机体炎症反应，改善神经功能缺损，安全性好。 |

| Objective To explore the clinical effect of Jiawei Lingguizhugan decoction combined with acupuncture in the treatment of stroke-related pneumonia and its influence on serum inflammatory factors.Methods One hundred and two hospitalized patients with stroke-related pneumonia in the Department of Rehabilitation, the Second Affiliated Hospital of Anhui Medical University fromJanuary 2018 to March 2020 were selected and randomly assigned into three groups: group A, group B and group C by using a randomnumber table. Group A was treated with conventional western medicine, group B was treated with Jiawei Lingguizhugan Decoction onthe basis of group A, while group C was treated with Jiawei Lingguizhugan Decoction and acupuncture on the basis of group A. The total clinical effective rate, the level of serum inflammation before and after treatment, the recovery of nerve function injury and adversereactions were analyzed.Results After treatment, the total clinical effective rate of group C (88.24%) was significantly higher than thatof group A (70.59%) and group B (76.47%)(P<0.05). Compared with before treatment, C-reactive protein (CRP), whitecell count (WBC),procalcitonin (PCT) and NIH Stroke Scale (NHISS) in the three groups decreased significantly after treatment (P<0.01); after treatment, WBC and NHISS of group C were significantly lower than that of group A and group B (P<0.01); CRP, PCT were also lower than that of the group A and group B, with a significant difference (P<0.05).Conclusion Jiawei Lingguizhugan decoction combined with acupuncture is effective in the treatment of stroke associated pneumonia, which can effectively reduce the inflammatory response of the body,improve the function of nerve function defect, and has good safety. |
