      目的探讨泛素特异性肽酶22(USP22)、滋养层细胞表面抗原-2(Trop2)表达与鼻咽癌(NPC)病人临床病理特征的关系及对预后的影响。方法选取2014年1月至2016年6月安阳市眼科医院NPC病人及其组织标本92例作为观察组,另选取同期慢性鼻咽炎病人及其组织标本95例作为对照组,检测对比两组USP22、Trop2蛋白表达水平及USP22、Trop2诊断价值。比较不同临床病理特征病人USP22、Trop2蛋白表达水平及其与NPC临床病理特征关联性。并对比不同USP22、Trop2表达水平病人3年无进展生存期及总生存期。结果观察组USP22(59.78%比12.63%)、Trop2(66.30%比15.79%)蛋白阳性表达率高于对照组(P<0.05);logistic回归显示:TNM分期、淋巴结转移、分化程度等3个因素均为USP22、Trop2蛋白表达的影响因素或关联因素(P<0.05);USP22、Trop2联合诊断准确性、敏感性高于二者单一诊断;TNM分期Ⅲ~Ⅳ期、低分化、淋巴结转移的NPC病人组织标本中USP22、Trop2蛋白阳性表达率高于TNM分期Ⅰ~Ⅱ期、中高分化、无淋巴结转移者(P<0.05);USP22、Trop2蛋白水平与TNM分期、淋巴结转移呈正相关,与分化程度呈负相关(P<0.05);USP22、Trop2蛋白阳性表达病人3年总生存率、无进展生存率低于阴性表达病人(P<0.05);USP22蛋白、Trop2蛋白高危组、低危组生存率对比,差异有统计学意义(P<0.05)。结论Trop2、USP22蛋白表达水平在NPC病人中呈异常高表达状态,与TNM分期、分化程度、淋巴结转移存在一定关联性,加强Trop2、USP22蛋白表达水平联合检测,对早期识别NPC、评估预后具有重要指导意义。

      Objective To investigate the relationship between the expression of ubiquitin specific peptidase 22 (USP22) and tropho?blast cell surface antigen-2 (Trop2) and the clinicopathological characteristics of nasopharyngeal carcinoma (NPC) patients and their impact on prognosis.Methods From January 2014 to June 2016, 92 NPC patients and their tissue samples from Anyang Eye Hospital were selected as the observation group, and 95 patients with chronic nasopharyngitis and their tissue samples during the same period were selected as the control group. The protein expression levels of USP22 and Trop2 in the two groups were detected and their diagnos?tic values were compared. The protein expression levels of USP22 and Trop2 in patients with different clinicopathological features and their correlation with clinicopathological features of NPC were compared. The 3-year progression-free survival and overall survival of patients with different USP22 and Trop2 expression levels were compared.Results The positive expression rates of USP22 (59.78%vs. 12.63%) and Trop2 (66.30% vs. 15.79%) protein in the observation group were higher than those in the control group(P<0.05).Logis?tic regression showed that three factors including TNM stage, lymph node metastasis and degree of differentiation were all influencing factors or associated factors of USP22 and Trop2 protein expression (P<0.05).The combined diagnosis accuracy and sensitivity of USP22 and Trop2 were higher than those of the two alone (P<0.05).The positive expression rate of USP22 and Trop2 protein in tissue samples of NPC patients with TNM stage Ⅲ~Ⅳ,poorly differentiated,lymph node metastasis was higher than that in TNM stage Ⅰ~Ⅱ,moderately and highly differentiated, and no lymph node metastasis (P<0.05).The protein levels of USP22 and Trop2 were positively correlated with TNM stage and lymph node metastasis and negatively correlated with the degree of differentiation (P<0.05).The 3-year overall survival rate and progression-free survival rate of patients with positive expression of USP22 and Trop2 protein were lower than those of patients with negative expression of USP22 and Trop2 proteins (P < 0.05).The survival rates of the USP22 protein, Trop2 pro? tein high-risk group and low-risk group were compared, and the difference was statistically significant (P < 0.05).Conclusions The protein expression levels of Trop2 and USP22 are abnormally high in NPC patients, and there is a certain correlation with TNM stage,degree of differentiation, and lymph node metastasis.Strengthening the combined detection of protein expression levels of Trop2 and USP22 has important guidance for early identification of NPC and evaluation of prognosis significance.
