      目的探讨急性支气管哮喘病人外周血程序性死亡因子 -1(PD-1)及其配体 PD-L1的表达水平及意义。方法选择 2017年 2月至 2019年 3月无锡市第五人民医院收治的 101例急性支气管哮喘病人作为病例组,同期体检正常 40例健康者为对照组;病例组病人分为轻中度组 46例、重度组 34例、危重组 21例。采用流式细胞术法检测外周血 CD4+T淋巴细胞中 PD-1表达( CD4+PD-1)、 CD8+T淋巴细胞中 PD-1表达( CD8+PD-1)CD19+B淋巴细胞中 PD-L1(CD19+PD-L1)表达,采用单因素方差分析其与疾病严重程度的关系;采用化学发光分析仪检测呼出,气一氧化氮( FeNO)水平,酶联免疫吸附法( ELISA)检测外周血白细胞介素 4(IL-4)、白细胞介素 17(IL-17)水平。结果病例组外周血 CD4+PD-1、CD19+PD-L1表达水平及 PD-1/PD-L1值均高于对照组高[ 17.98±4.16比 5.43±1.52、3.13±0.75比 1.08±0.27、5.75±1.62比 5.03±1.11](P<0.05);病例组 FeNO及外周血 IL-17水平高于对照组[( 29.74±6.08)μg/L比( 16.38±4.12)μg/L、(38.66±8.27)ng/L比( 24.73±5.21)ng/L],外周血 IL-4水平低于对照组[(9.14±2.47)ng/L比( 12.86±3.65)ng/L](P<0.05);病例组不同严重程度外周血 CD4+PD-1、CD19+PD-L1表达水平及 PD-1/PD-L1值由高至低为危重组、重度组、轻中度组(P<0.05);病例组外周血 CD4+PD-1、CD19+PD-L1与 FeNO呈正相关关系(r=0.61、0.55, P<0.05)与 IL-17呈正相关关系( r=0.51、0.50,P<0.05)与 IL-4呈负相关关系(r=.0.43、.0.38,P<0.05)。结论急性支气管哮喘病人外周血,PD-1/PD-L1(CD4+PD-1/CD19+PD-L1)表达异,常,病情越严重,其表达水平可能越高。

      Objective To investigate the expression and significance of programmed death factor-1 (PD-1) and its ligand PD-L1 in peripheral blood of patients with acute bronchial asthma.Methods Totally 101 cases of acute bronchial asthma admitted to WuxiFifth People's Hospital from February 2017 to March 2019 were selected as case group, and 40 healthy subjects were enrolled in thecontrol group. Patients in the case group were subdivided into mild to moderate group (n = 46), severe group (n = 34) and critical group (n = 21). The expressions of PD-1 in peripheral blood CD4+T lymphocytes (CD4+PD-1), CD8+T lymphocytes (CD8+PD-1), and CD19+ B lymphocytes (CD19+PD-L1) were detected by flow cytometry and the relationship between PD-1 expression and disease severity was analyzed by one-way ANOVA. The level of exhaled nitric oxide (FeNO) was detected by chemiluminescence analyzer, and the levels of interleukin-4 (IL-4) and interleukin-17 (IL-17) in peripheral blood were detected by enzyme-linked immunosorbent assay (ELISA).Re. sults The expression levels of CD4+PD-1, CD19+PD-L1 and PD-1/PD-L1 in peripheral blood of the case group was higher than those of the control group [(17.98±4.16) vs. (5.43±1.52), (3.13±0.75) vs. (1.08±0.27), (5.75±1.62) vs. (5.03±1.11); P < 0.05]. The FeNO level and peripheral blood IL-17 level in the case group were higher than those in the control group [(29.74±6.08) μg/L vs. (16.38±4.12) μg/ L, (38.66±8.27) ng/L vs. (24.73±5.21) ng/L; P < 0.05], while the level of IL-4 in the case group was lower than that in the control group [(9.14±2.47) ng/L vs. (12.86±3.65) ng/L, P < 0.05]. The expression levels of CD4+PD-1, CD19+PD-L1 and PD-1/PD-L1 in peripheral blood of patients with different severity from high to low were found in critical group, severe group and mild-to-moderate group (P < 0.05). In the case group, CD4+PD-1 and CD19+PD-L1 were positively correlated with FeNO (r = 0.61, 0.55, respectively; P < 0.05), positively correlated with IL-17 (r = 0.51, 0.50, respectively; P < 0.05), and negatively correlated with IL-4(r = .0.43, .0.38, respec. tively; P < 0.05).Conclusion The expression of PD-1/PD-L1 (CD4+PD-1 / CD19+PD-L1) in peripheral blood of patients with acutebronchial asthma is abnormal. The more serious the disease is, the higher the expression level may be.
