| 目的 研究不同糖耐量冠心病(CAD)病人临床指标、冠脉造影特点。方法 选取明确诊断冠心病的病人230例,通过糖尿病病史及口服糖耐量试验(OGTT)结果分为血糖正常组(NGT)(n=84)、糖耐量受损组(IGT)(n=66)、糖尿病组(DM)(n=80),收集三组病人临床指标及分析冠状动脉病变的差异。结果 (1)三组在平均年龄、体质量指数(BMI)、血压比较方面差异无统计学意义(P>0.05)；(2) NGT组平均三酰甘油(TG)1.78 mmol·L-1,胆固醇(TC)为4.18 mmol·L-1,低密度脂蛋白(LDL-C)均值2.56 mg·L-1,尿微量白蛋白均值28.09 mg·L-1,与DM及IGT组相比,差异有统计学意义(P<0.05)；(3)三组冠状动脉病变支数、部位和狭窄程度相比差异有统计学意义(P<0.05) 。结论 糖耐量异常或糖尿病均加重冠状动脉病变程度,空腹血糖正常的CAD病人均应行OGTT检查,降低糖耐量异常的漏诊率。 |

| Objective To investigate the characteristics of the coronary heart disease with different glucose tolerance in clinical presentation and coronary angiography.Methods A total of 230 patients with coronary heart disease were assigned into 3 groups according to history of diabetes mellitus and results of oral glucose tolerance test (OGTT):normal plasma glucose (NGT) group (n=84),impaired glucose tolerance (IGT) group (n=66),and diabetes mellitus (DM) group (n=80).The difference in clinical data and coronary artery lesions among the three groups were compared.Results (1) There were no differences in age,body mass indices,blood pressure among the three groups (P>0.05).(2) In NGT group,on average,triglycerides (TG),cholesterol (TC) ,low density lipoprotein (LDL-C) and urine trace albumin were 1.78 mmol·L-1,4.18 mmol·L-1,2.56 mg·L-1,and 28.09 mg·L-1,respectively,which were significantly different compared with group DM and IGT (P<0.05).(3) There were significant differences in coronary artery lesion counts,location of lesions,and degree of stenosis among three groups (P<0.05).Conclusions IGT or DM can both aggravate the degree of coronary lesion.CAD patients with normal glucose all should take OGTT in order not to miss those patients with abnormal glucose tolerance. |
