      目的 观察缺血性心肌病型心衰病人标准化治疗方案下,辅助运用参麦注射液对左室射血功能和心肺运动能力的影响。方法 轻中度缺血性心肌病型心衰病人133例,随机数字表法分为观察组(标准化治疗联合参麦注射液)和对照组(标准化治疗联合安慰剂),分别评估治疗前与治疗12个月后超声心动图指标和心肺运动试验结果。结果 12个月后随访,观察组和对照组多项心脏彩超和心肺运动试验参数均差异有统计学意义(P<0.001):(1)左室射血分数(分别为增加10.4%和降低5.0%);(2)峰值耗氧量(分别为增加6.2%和降低4.5%);(3)运动时间(分别为增加7.5%和降低4.8%);观察组和对照组心衰住院率分别为6%和30% (P=0.0002)。结论 对于轻中度缺血性心肌病型心衰病人,在以循证医学为基础的标准化治疗上联合运用参麦注射液,可以增强左室射血功能及心肺运动能力,并降低住院率。
英文摘要:
      Objective To observe the effects of ShenmaiInjection(SMI) on left ventricular (LV) systolic function in chronic heart failure (HF) due to ischemic cardiomyopathy (ICM).Methods One hundred and thirty-three patients of HF with ICM and minimal symptoms were randomized into SMI group (standard therapy plus Shenmai injection) and placebo group (standard therapy plus placebo).LV function and functional capacity were assessed prospectively by echocardiography and cardiopulmonary exercise testing at baseline and at 12 months after randomization.Results At 12 months after randomization,the SMI group and the placebo group differed significantly (P<0.001) in regard to:(1) LV ejection fraction (increased by 10.4% and decreased by 5.0%,respectively);(2) peak VO2(increased by 6.2% and decreased by 4.5%,respectively);(3) exercise duration (increased by 7.5% and decreased by 4.8%,respectively).The hospitalization rates for HF were 6% in the SMI and 30% in the placebo group (P=0.0002).Conclusions In patients with ICM and minimal symptoms in response to evidence-based medical therapy,SMI treatment increases LV systolic function and functional capacity and may reduce hospitalizations for HF.
