| 目的 探讨股骨近端防旋髓内钉(PFNA)治疗高龄股骨粗隆间骨折的技术要点和注意事项。方法 选择进行手术内固定治疗的40例高龄股骨粗隆间骨折病人进行研究,观察临床治疗效果。结果 所有病人均获得了随访,平均随访时间8个月,术中平均手术时间(41.73±2.52) min,平均出血量(69.26±15.14) mL,对病人进行Harris髋关节功能评分,评分结果:优26例,良6例,中5例,差3例,优良率80.0%。术后定期复查X线片,骨折临床愈合时间8～14周。病人在术后出现了髋内翻3例,股骨近端轻度劈裂4例,并有1例病人出现了轻微的下肢深静脉血栓等并发症。结论 采用PFNA治疗高龄股骨粗隆间骨折不仅操作简单快捷,而且治疗效果较好、并发症较少,值得在临床上推广使用。 |

| Objective To investigate the technical key point and precautions of proximal femoral nail antirotation (PFNA) for the treatment of femoral intertrochanteric fractures in elderly patients.Methods 40 cases of elderly patients with femoral intertrochanteric fractures treated with internal fixation were collected from our hospital.Results All cases were followed-up with the average time of 8 months.The mean operative time was (41.73±2.52) min,the mean intraoperative blood loss was (69.26±15.14) mL.The results of the Harris hip score were excellent in 26 cases,good in 6 cases,fair in 5 cases and poor in 3 cases,the excellent and good rate was 80.0%.The X-ray were regularly reviewed after surgery and the average healing time was 8-14 weeks.There were 3 cases of coxa varus,4 cases of mild split in the proximal femur and 1 case of complications such as deep venous thrombosis of the lower extremity.Conclusion The treatment of femoral intertrochanteric fractures with PFNA in elderly patients is not only simple and fast,but also has better therapeutic effect and less complications. |
