      目的 探索免疫组化法检测肺腺癌病人表皮生长因子受体-19、21(EGFR-19、21)突变和甲状腺转录因子-1(TTF-1)表达与临床特征的关系。方法 选取肺腺癌病人114例,通过免疫组化检测EGFR-19、21的突变和TTF-1的表达,分析EGFR-19、21突变、TTF-1表达与病人临床特征的关系。结果 EGFR-19在中-高分化的肺腺癌中阳性表达率较高,EGFR-21在中-高分化、不吸烟的肺腺癌中阳性表达率较高,TTF-1在女性、不吸烟的肺腺癌阳性表达率较高,TTF-1阳性的表达与EGFR-21突变具有相关性。结论 在肺腺癌病人中,对可能从靶向治疗中获益但却难以开展基因检测EGFR突变的地区,可将TTF-1表达阳性联合EGFR-21突变伴有不吸烟等临床特征,预测肺腺癌EGFR-21的突变情况,为可能从靶向治疗中获益肺腺癌病人提供参考。
英文摘要:
      Objective To explore the correlation of EGFR-19,EGFR-21 mutations and TTF-1 expression with the clinical features in patients with lung adenocarcinoma by immunohistochemistry.Methods In 114 cases of lung adenocarcinoma,the mutations of EGFR-19,1 and the expression of TTF-1 were detected by immunohistochemistry,meanwhile the relation of EGFR-19,1 mutations,TTF-1 expression and the clinical features in patients were analyzed.Results The positive rate of EGFR-19 expression was higher in the patients with moderate and high differentiated lung adenocarcinoma,and the positive rate of EGFR-21 expression was higher in high differentiation lung adenocarcinoma patients and non-smokers.TTF-1 in female patients and non-smokers had a higher positive rate.And the positive expression rate of TTF-1 was related to the positive mutation rate of EGFR-21.Conclusions The patients with lung adenocarcinoma might benefits from targeted therapy,however it is difficult to carry out the regional gene detection of EGFR mutation in the areas where they live.It can predicts the EGFR-21 mutations in lung adenocarcinoma patients through analyzing the clinical features of the TTF-1 positive expression and the EGFR-21 mutation in non-smokers,so that it provides a reference for patients with lung adenocarcinoma to get a targeted therapy.
