      目的 观察沙格列汀对早期糖尿病肾病患者的治疗效果。方法 80例早期糖尿病肾病患者,采用随机数字表法分为沙格列汀组与阿卡波糖组,所有患者均予以门冬胰岛素30注射液,皮下注射治疗,2次/天(早晚餐时),治疗组同时口服沙格列汀片 5 mg·d-1,对照组同时口服阿卡波糖100~200 mg·d-1,分别于治疗前(0周) 及治疗后第24周测定空腹血糖(FBG)、餐后2 h血糖(2 h PBG)、糖化血红蛋白(HbA1C),高敏C-反应蛋白(hs-CRP)、白细胞介素-6(IL-6)、尿蛋白排泄率(UAER)。结果 两组患者治疗24周后,与治疗前相比,FBG、2 h PBG、HbA1C均明显下降(P<0.001);阿卡波糖组24 h UAER较治疗前有所下降,但差异无统计学意义,沙格列汀组24 h UAER较治疗前明显下降,差异有统计学意义(P<0.001),两组间比较差异有统计学意义(P<0.001);阿卡波糖组hs-CRP、IL-6较治疗前有所下降,但差异无统计学意义,沙格列汀组hs-CRP、IL-6较治疗前明显下降,差异有统计学意义(P<0.001),两组间比较差异有统计学意义(P<0.001)。结论 沙格列汀能降低早期糖尿病肾病患者的24 h UAER,它的抗炎作用可能是其治疗糖尿病肾病的机制之一。
英文摘要:
      Objective To evaluate the effects of saxagliptin on patients with early diabetes nephropathy. Methods A total of 80 cases of diabetes nephropathy at early stage were randomly assigned into saxagliptin group and acarbose group.The saxagliptin group was given insulin aspart and oral saxagliptin while the acarbose group was given insulin aspart and oral acarbose.The change of fasting blood glucose(FBG),2 h postprandial blood glucose(2 h PBG),glycosylated hemoglobin A(HbA1C),high sensitivity Creactive protein(hs-CRP),interleukin-6(IL-6) and urinary albumin excretion rate(UAER) were evaluated after 24 weeks′ treatment. Results Over the 24 weeks treatment period,FBG,2 h PBG,HbA1C,hs-CRP,IL-6 and UAER of two groups were significantly decreased than base line(P<0.001).UAER of acarbose group was decreased than before treatment,with no significant difference.UAER of saxagliptin group was decreased than before treatment,with the difference was significant(P<0.001).The change of UAER has statistically significant difference between the two groups(P<0.001).hs-CRP,IL-6 levels of acarbose group were not decreased after treatment.hs-CRP,IL-6 levels of saxagliptin group were significantly decreased after treatment(P<0.001).The changes of hs-CRP,IL-6 levels have statistically significant differences between the two groups(P<0.001). Conclusions Saxagliptin can effectively reduce UAER in patients with early diabetes nephropathy,which anti-inflammatory effects might be one of the protection mechanism of diabetic nephropathy.
