| 目的 分析该院临床不合理用药医嘱情况,为临床安全、有效、合理用药提供参考。方法 对2015年7月—2016年7月住院药房不合理用药医嘱进行统计分析。结果 不合理医嘱共9 408条,占总医嘱的1.94%,类型包括药物配伍不合理、溶剂选择不合理、给药剂量不合理、药物浓度不合理、给药频次不合理、给药途径不合理与录入错误。结论 通过对静脉医嘱进行审核和干预,能及时发现并纠正临床不合理用药现象,促进临床合理用药。 |

| Objective To analyze the situation of the irrational medication orders in our hospital and to provide reference for clinical safe,effective and rational drug use. Methods The irrational medication orders of the inpatient pharmacy in Department of Pharmacy,Anhui Provincial Hospital from July 2015 to July 2016 were analyzed statistically. Results There were 9 408 irrational medication orders,accounting for 1.94%,including drug incompatibility,irrational solvent,irrational dose,irrational concentration,irrational medication frequency and entry errors. Conclusions The review and intervention on the medication orders,able to discover and correct irrational drug use can promote the clinical rational medication. |
