| 目的探讨全身低剂量 CT检查在多发性骨髓瘤病人诊断及分期中的应用价值。方法收集 2016年 10月至 2018年 10月保定市第一中心医院血液科 58例经临床病理证实的多发性骨髓瘤病人的全身低剂量 CT检查的影像学资料，以此进行 Durie/Salmon plus分期，同时进行临床 ISS分期，比较两者在溶骨性病灶的差异性和一致性。结果 58例多发性骨髓瘤病人共发现 558处溶骨性病灶，其中 Durie/Salmon plus分期: Ⅰ期 12例， Ⅱ期 32例， Ⅲ期 14例；临床 ISS分期: Ⅰ期 18例， Ⅱ期 21例， Ⅲ期 19例。两种分期相比差异无统计学意义( χ2＝4.241，P＝0.120)Kappa值为 0.423，具有中等程度的一致性。结论 Durie/ Salmon plus分期和 ISS分期之间差异无统计学意义，全身低剂量 CT检，查在多发性骨髓瘤诊断及分期中有良好的应用价值。 |

| Objective To investigate the value of whole?body low?dose CT in the diagnosis and staging of patients with multiple my? eloma.Methods The imaging data of 58 patients with multiple myeloma confirmed by clinical pathology in the Department of He?matology of The First Central Hospital of Baoding from October 2016 to October 2018 were collected and separated by the Durie/Salmon plus staging and the clinical ISS staging.The difference and consistency between them in osteolytic lesions were compared.Results A total of 558 osteolytic lesions were found in the 58 patients with multiple myeloma，including Durie/Salmon plus stage Ⅰ(n＝12)stage Ⅱ(n＝32)and stage Ⅲ(n＝14)，and clinical ISS stage:stage Ⅰ(n＝18)，stage Ⅱ(n＝21)and stage Ⅲ(n＝19).Therew，as no significant difference between the two stages(χ2＝4.241，P＝0.120)，and the Kappa value was 0.423，which was moderately consistent.Conclusion There is no significant difference between Durie/Salmon plus staging and ISS staging.Thewhole?body low?dose computed tomography has good application value in the diagnosis and staging of multiple myeloma. |
