| 目的系统评价超声辅助与传统盲探在椎管内麻醉应用的安全性与有效性，为临床超声可视化下进行椎管内麻醉提供参考。方法检索中文期刊全文数据库( CNKI)、重庆维普( VIP)、万方数据库， Pubmed，Elsevier，查找有关超声辅助与传统盲探在椎管内麻醉应用的随机对照试验，检索时限截止到 2018年 10月，并采用 stata15.0统计软件进行 meta分析。结果共 33篇研究文献入选。 Meta分析结果表明，与传统盲探组比较，超声辅助组穿刺并发症发生率低( RR＝0.481，95%CI:0.376～ 0.614，P＜0.001)，穿刺次数较少( WMD＝-0.938，95%CI:-1.069~-0.808，P＜0.001)首次穿刺成功率较高( RR＝1.240，95%CI1.132～1.357，P＜0.001)，穿刺时间较少( WMD＝-2.253，95%CI:-2.936～-1.570，P＜0.001)，然而定位时间较长( WMD＝2.559，95%CI:1.167～3.951，P＜0.001)。结论超声辅助下的椎管内麻醉能够显著提高椎管内麻醉的有效性及安全性，值得推广，但该结论尚需进一步研究进行验证。 |

| Objective To systematically evaluate the safety and effectiveness of ultrasound?assisted and traditional blind spinal an? esthesia，and to provide reference for clinical intraspinal anesthesia under ultrasound visualization.Methods China Knowledge Re? source Integrated Database(CNKI)，Chongqing VIP(VIP)，Wanfang Data，Pubmed，Cochrane Library，Elsevier were searched for randomized controlled trials of ultrasound?assisted and traditional blind spinal anesthesia.The search deadline was October 2018， and meta?analysis was performed using stata15.0 statistical software.Results A total of 33 studies were included.Meta?analysis showed that compared with traditional blind group，ultrasound?assisted group had lower incidence of puncture complications(RR＝ 0.481，95%CI:0.376?0.614，P＜0.001)，fewer puncture times(WMD＝-0.938，95%CI:-1.069?-0.808，P＜0.001)，higher success rate of the first puncture(RR＝1.240，95%CI:1.132?1.357，P＜0.001)，less procedure time(WMD＝-2.253，95%CI:-2.936?-1.570， P＜0.001)and longer positioning time(WMD＝2.559，95%CI:1.167?3.951，P＜0.001)Conclusion Ultrasound?assisted spinal anesthesia can significantly improve the effectiveness and safety of spinal anesthesia，which is worthy of promotion.However，this |
