      目的分析医院住院病人血培养阳性病原菌分布特征及其耐药性,为临床选择有效抗菌药物提供参考依据。方法回顾性分析 2014年 1月至 2018年 12月亳州市人民医院血培养阳性病人的病原菌分布特点及药敏结果。结果收集 4年间血培养标本中,分离出 2 240株病原菌,革兰阳性球菌 1 173株( 52.37%)以凝固酶阴性葡萄球菌、金黄色葡萄球菌为主,暂未发现对利奈唑胺、替加环素及万古霉素耐药。革兰阴性菌 1 037株( 46.29%),以大肠埃希菌、肺炎克雷伯杆菌为主,其中产 ESBL阳性菌 353株,检出率 52.45%,对加酶抑制剂、头孢替坦、碳青霉烯类、阿米卡星的敏感性较好,但鲍曼不动杆菌耐药菌较严重。酵母样菌株 30株,对 5?氟胞嘧啶、两性霉素 B、氟康唑、伊曲康唑、伏立康唑均敏感。儿科( 652株)、重症监护室( 428株)、感染科( 165株)、血液科( 156株)、呼吸科( 138株)血培养阳性排在临床科室前 5位。结论血培养病原菌耐药率不断发生变化;临床医生及时了解血培养中细菌的分布特征及耐药性,对减少院内感染、指导临床规范合理使用抗菌药物有重要意义。

      Objective To analyze the distribution characteristics and drug resistance of blood culture?positive pathogen in hospital? ized patients,and to provide reference for clinical selection of effective antibacterial drugs.Methods Retrospective analysis of thedistribution characteristics and drug susceptibility results of blood culture?positive pathogens in the People’s Hospital of Bozhou from Janurary 2014 to December 2018.Results A total of 2 240 strains of pathogens were isolated from blood culture specimens collected during 4 years,of which 1 173 strains(52.37%)were Gram?positive cocci,mainly coagulase?negative staphylococci and Staphylococcus aureus,and no resistance to linezolid,tigecycline and vancomycin was found.There were 1 037 strains(46.29%)of Gram?negative bacteria,mainly Escherichia coli and Klebsiella pneumoniae,among which 353 strains were ESBL?positive bacteria were,the detection rate was 52.45%.They were sensitive to enzyme inhibitors,cefotetan,carbapenems and amikacin,but resistance of acinetobacter baumannii was serious.There were 30 yeast?like strains,which were sensitive to 5?fluorocytosine,amphotericin B, fluconazole,itraconazole,and voriconazole.Pediatrics(652 strains), intensive care units(428 strains), infectious diseases(165 strains),hematology(156 strains),and respiratory departments(138 strains)were positively ranked in the top five clinical depart? ments.Conclusions The drug resistance rate of blood culture pathogens is constantly changing.Clinicians should keep abreast ofthe distribution characteristics and drug resistance of bacteria in blood culture,which is of great significance for reducing nosocomi?al infections and guiding clinical rational and standardized use of antibacterial drugs.
