      目的分析围绝经期糖代谢异常升高与骨代谢指标的相关性,并探讨其临床意义。方法选取徐州市中心医院新城区分院 2018年 1—12月收治的围绝经期女性病人 100例,其中围绝经期糖代谢异常升高 50例为血糖异常组,围绝经期糖代谢正常 50例为血糖正常组。比较两组血糖水平、骨形成指标[骨碱性磷酸酶(BALP)、骨钙素(BGP)]、骨吸收指标[Ⅰ型胶原联蛋白羧基末端肽(CTX1)、 I型胶原氨基酸延长肽(tPINP)]、骨钙调节素(PTH)指标,并采用 pearson相关法分析血糖异常组血糖水平与骨代谢指标的相关性。结果血糖异常组的 BALP低于血糖正常组[(73.59±8.00)U/L比(96.33±12.55)U/L,t=4.997,P= 0.000],血糖异常组的 BGP[(21.06±2.19)ng/mL比(12.23±1.44)ng/mL]、 CTX1[(56.63±3.79)ng/L比(41.67±2.13)ng/L]、 tPINP[(55.76±3.78)ng/mL比(40.37±2.50)ng/mL]、 PTH[(71.60±6.34)ng/L比(44.56±3.84)ng/L]均高于血糖正常组(P<0.05)。空腹血糖对比,血糖异常组高于血糖正常组[(9.39±1.99)mmol/L比(4.21±1.01)mmol/L,t=24.164,P=0.000]。血糖异常升高妇女的血糖水平与 BALP呈负相关(r=-0.850,P=0.000),与 BGP呈正相关(r=0.867,P=0.000)、与 CTX1呈正相关(r=0.882,P=0.000),与 tPINP呈正相关(r=0.881,P=0.000)、与 PTH呈正相关(r=0.923,P=0.000)。结论围绝经期糖代谢异常升高妇女的骨代谢指标存在一定的异常,且血糖水平异常升高可能是围绝经期妇女骨代谢异常引起骨质疏松症的重要原因。

      Objective To analyze the correlation between abnormal increase of glucose metabolism and bone metabolism in peri? menopausal women,and to explore its clinical significance.Methods A total of 100 perimenopausal female patients admitted tothe Xincheng Branch of Xuzhou Central Hospital from January to December 2018 were selected.Among them,50 perimenopausalwomen with abnormal elevation of glucose metabolism and 50 perimenopausal women with normal glucose metabolism.Blood sugarlevel,bone formation index(BALP,BGP),bone resorption index(CTX1,tPINP)and osteocalcin(PTH)were measured and com? pared between the two groups.Pearson correlation method was used to analyze the correlation between blood sugar level and bonemetabolism index in women with abnormal blood sugar.Results Compared with the normal blood glucose group,the BALP of the abnormal blood glucose group was lower than that of the normal blood glucose group[(73.59±8.00)U/L vs.(96.33±12.55)U/L,t=4.997,P=0.000]and the BGP[(21.06±2.19)ng/mL vs.(12.23±1.44)ng/mL]CTX1[(56.63±3.79)ng/ L vs.(41.67±2.13)ng/ L], tPINP[(55.76±3.78)n,g/mL vs.(40.37±2.50)ng/mL],PTH[(71.60±6.34)ng/L vs.(,44.56±3.84)ng/L]of the abnormal blood glucose group were higher than those of the normal blood glucose group(P<0.05).Compared with the normal group,the abnormal group was higher than the normal group[(9.39±1.99)mmol/L vs.(4.21±1.01)mmol/L,t=24.164,P=0.000].The blood sugar level of wom? en with abnormal elevation of blood sugar was negatively correlated with BALP(r=-0.850,P=0.000),positively correlated with BGP(r=0.867,P=0.000),positively correlated with CTX1(r=0.882,P=0.000),positively correlated with tPINP(r=0.881,P= 0.000),and positively correlated with PTH(r=0.923,P=0.000).Conclusion There are some abnormal bone metabolic indexes in perimenopausal women with abnormal elevation of glucose metabolism,and abnormal elevation of blood sugar levels may be animportant cause of osteoporosis caused by abnormal bone metabolism in perimenopausal women.There are certain abnormalities inbone metabolism indexes in women with abnormally elevated glucose metabolism in perimenopausal women,and abnormally elevat?ed blood glucose levels may be an important cause of fractures caused by abnormal bone metabolism in perimenopausal women.
