      目的探讨膝骨关节炎( KOA)软骨下骨髓病变( BMLs)的磁共振成像评分与其临床表现的相关性。方法纳入 2015年 3月至 2018年 3月辽宁省金秋医院收治的 KOA病人 52例作为研究对象, BMLs采用整体器官磁共振成像( WORMS)评分进行评价,临床表现采用西安大略和麦克马斯特大学骨关节炎指数( WOMAC)进行评分评价,运用 Spearman秩相关分析对 WORMS评分、 WOMAC评分的相关性进行分析。采用多元线性回归分析各因素对于 WORMS评分的影响。结果 52例病人中, WORMS评分 0分病人有 6例, 1分病人有 17例, 2分病人有 21例, 3分病人有 8例。依据 WORMS评分对病人进行分组, 0分组至 3分组 WOMAC总评分分别为( 47.82±3.66)分、(65.78±6.44)分、(102.43±10.54)分、(111.07±15.64)分,各组间 WOMAC评分比较差异有统计学意义( F=0.86,P<0.001)。 BMLs的 WORMS评分与临床表现的 WOMAC总评分及各分项评分均呈正相关性( P<0.05),其中与疼痛项目评分相关性最为显著( rs=0.947,P<0.001)且疼痛项目评分及身体功能评分是影响 WORMS评分的两个因素( P<0.05)。结论软骨下 BMLs程度与 KOA病人临床表现呈,正相关,磁共振成像检查在 BMLs的评估和分级中具有重要的参考价值。

      Objective To investigate the correlation between whole organ magnetic resonance imaging score (WORMS) and the clinical manifestations in patients with subchondral bone marrow lesions (BMLs) associated with knee osteoarthritis (KOA).Methods Fifty-two KOA patients, treated in Liaoning Jinqiu Hospital from March 2015 to March 2018, were included. The imaging severity ofBMLs was evaluated by WORMS, and the clinical manifestations were evaluated with Western Ontario and McMaster University Osteoarthritis Index (WOMAC). Then, the Spearman′s rank correlation was used to analyze their correlation. The multiple linear regressionanalysis was used to analyze the influence of various factors on WORMS.Results Among the 52 patients, 6 patients had a WORMS of0, 17 a score of 1, 21 a score of 2, and 8 a score of 3. According to the WORMS scoring, the patients were divided into groups. The totalWOMAC scores from group with a score of 0 to group with a score of 3 were (47.82±3.66) points, (65.78±6.44) points, (102.43±10.54)points, and (111.07±15.64) points respectively. There was a statistically significant difference in the WOMAC score among groups (F= 0.86, P<0.001). The WORMS score of BMLs was positively correlated with the WOMAC total score and each sub-score of clinical manifestations (P<0.05), and the correlation with the pain item score was the most significant (rs=0.947, P<0.001). Pain item score and physical function score were the influencing factors of WORMS (P<0.05).Conclusions The severity of subchondral BMLs was positivelycorrelated with the clinical manifestations of KOA patients. MRI examination is of great value in the evaluation and grading of BMLs.
