      目的探究宫颈鳞癌组织中泛素偶联酶E2(UBE2C)、食管癌相关基因4(ECRG4)表达水平及其与病人预后关系。方法选取2010年6月至2014年6月天门市第一人民医院收治的宫颈鳞癌病人73例、宫颈上皮瘤变病人62例、因子宫肌瘤需行全子宫切除术病人54例为研究对象。术中分别收集宫颈鳞癌病人宫颈鳞癌组织、宫颈上皮瘤变病人瘤变组织和子宫肌瘤病人正常宫颈组织。分别采用实时荧光定量逆转录聚合酶链式反应(qRT-PCR)和免疫组织化学染色法检测UBE2C、ECRG4mRNA和蛋白表达水平。分析二者表达水平与病人临床病理特征及预后关系。结果正常宫颈组织、宫颈上皮内瘤变组织、宫颈鳞癌组织中UBE2C mRNA[(1.01±0.29)、(1.97±0.63)、(3.64±1.21)]表达水平、UBE2C 蛋白高表达率(20.37%、51.61%、68.49%)依次显著升高(P<0.05),ECRG4 mRNA[(1.02±0.31)、(0.75±0.26)、(0.51±0.19)]表达水平、ECRG4 蛋白高表达率(66.67%、43.55%、26.03%)依次显著降低(P<0.05)。UBE2C、ECRG4蛋白在低、中、高分化宫颈鳞癌病人癌组织中高表达率比较,差异有统计学意义(P<0.05)。UBE2C、ECRG4蛋白在Ⅰ~Ⅱ、Ⅲ期FIGO分期及有无淋巴结转移病人癌组织中高表达率比较,差异有统计学意义(P<0.05)。宫颈鳞癌组织中UBE2C mRNA与ECRG4 mRNA表达水平呈负相关(P<0.05)。UBE2C蛋白高表达组病人5年总生存率明显低于UBE2C蛋白低表达组,ECRG4蛋白高表达组病人5年总生存率明显高于ECRG4蛋白低表达组,均差异有统计学意义(34.00%比65.22%,73.68%比33.33%,P<0.05)。FIGO分期Ⅲ期、淋巴结转移、UBE2C高表达、ECRG4低表达均是影响宫颈鳞癌病人发生不良预后的独立危险因素(P<0.05)。结论宫颈鳞癌组织中UBE2C mRNA及蛋白表达水平明显升高,ECRG4 mRNA及蛋白表达水平明显降低,与病人5年总生存率低有关,可能是影响宫颈鳞癌病人发生不良预后的独立危险因素。

      Objective To investigate the expression levels of ubiquitin coupling enzyme E2 (UBE2C) and esophageal cancer-related gene 4 (ECRG4) in cervical squamous cell carcinoma and their relationships with prognosis.Methods Seventy-three cases of cervical squamous cell carcinoma, 62 cases of cervical epithelial neoplasia and 54 cases of myoma requiring total hysterectomy in the First Peo-ple's Hospital of Tianmen from June 2010 to June 2014 were selected as the research objects. During the operation, the cervical squa-mous cell carcinoma tissues, cervical epithelial neoplasia tissues and normal cervical myoma tissues were collected. The mRNA and protein expression levels of UBE2C and ECRG4 were detected by qRT-PCR and immunohistochemistry respectively. And the relation-ships between the expression levels of the two and the clinicopathological characteristics, prognosis of the patients were analyzed.Re-sults The expression level of UBE2C mRNA in normal cervical tissues, cervical intraepithelial neoplasia tissues and cervical squa-mous cell carcinoma tissues was (1.01±0.29), (1.97±0.63) and (3.64±1.21), respectively, and the high expression rate of UBE2C protein(20.37%, 51.61%, 68.49%) increased significantly in turn (P<0.05), while the expression level of ECRG4 mRNA was (1.02±0.31),(0.75±0.26) and (0.51±0.19), respectively, and the high expression rate of ECRG4 protein (66.67%, 43.55%, 26.03%) decreased signifi-cantly in turn (P<0.05). The high expression rates of UBE2C and ECRG4 proteins in the cancer tissues of patients with low, medium and highly differentiated cervical squamous cell carcinoma were compared, and the difference was statistically significant (P<0.05).The high expression rates of UBE2C, ECRG4 protein in carcinoma tissue of patients with Ⅰ,Ⅱ,Ⅲ stage FIGO staging, and with or without lymph node metastasis were compared, the difference was statistically significant (P<0.05). UBE2C mRNA was negatively cor-related with ECRG4 mRNA expression in cervical squamous cell carcinoma (P<0.05).The 5-year overall survival rate of patients withm high expression of UBE2C protein was significantly lower than that of patients with low expression of UBE2C protein, and the 5-year overall survival rate of patients with high expression of ECRG4 protein was significantly higher than that of patients with low expression of ECRG4 protein, and the difference was statistically significant (34.00% vs. 65.22%, 73.68% vs. 33.33%, P<0.05). FIGO stage Ⅲ,lymph node metastasis, UBE2C high expression and ECRG4 low expression were all independent risk factors for poor prognosis of cer-vical squamous cell carcinoma (P< 0.05).Conclusion The expression level of UBE2C mRNA and protein are significantly increased in cervical squamous cell carcinoma, while ECRG4 mRNA expression level and protein are decreased, they are related to the low 5-year overall survival rate of patients, and are independent risk factors for the poor prognosis of patients with cervical squamous cell car-cinoma.
