| 目的探究参芪扶正注射液治疗癌症相关疲劳病人疗效及对细胞免疫、转化生长因子-β1(TGF-β1)、肿瘤坏死因子-α(TNF-α)表达的影响。方法选取北京中医药大学东直门医院2018年9月至2019年9月癌症相关疲劳病人108例，按照随机数字表法分为观察组与对照组，各54例。对照组采取对症治疗，观察组在对照组基础上给予参芪扶正注射液治疗。比较两组治疗2周后总有效率、治疗前后中医症状积分、细胞免疫(CD3+、CD4+、CD8+)、血清TGF-β1、TNF-α水平、Piper疲乏量表评分及不良反应发生情况。结果观察组总有效率(87.04%)高于对照组(66.67%)(P<0.05)；治疗2周后，观察组气短、乏力、神疲积分低于对照组(P<0.05)；观察组治疗2周后CD3+、CD4+高于对照组，CD8+低于对照组(P<0.05)；观察组治疗2周后血清TGF-β1、TNF-α水平低于对照组(P<0.05)；观察组治疗2周后躯体疲乏、情感疲乏、行为疲乏、认知疲乏评分及疲乏总分低于对照组(P<0.05)；观察组不良反应发生率(12.96%)与对照组(16.67%)相比，差异无统计学意义(P>0.05)。结论参芪扶正注射液治疗CDF，能增强细胞免疫功能，有效改善病人临床症状，提高治疗效果，安全可靠，其抗CRF分子水平的机制可能与降低TGF-β1、TNF-α表达有关。 |

| Objective To explore the therapeutic effect of Shenqi Fuzheng injection on cancer-related fatigue and its effect on cellu?lar immunity, TGF-β1, TNF-α expression.Methods A total of 108 patients with cancer-related fatigue from September 2018 to Sep?tember 2019 in Dongzhimen Hospital, Beijing University of Traditional Chinese Medicine were selected and grouped according to the random number table method, 54 cases each. The control group took symptomatic treatment, and the observation group was given Shen?qi Fuzheng injection on the basis of the control group. The total effective rate after 2 weeks of treatment, TCM symptom score, cellular immunity (CD3+, CD4+, CD8+), serum TGF-β1, TNF-α levels, piper fatigue scale scores before and after treatment and adverse reac?tions were compared between the two groups.Results The total effective rate of the observation group (87.04%) was higher than that of the control group (66.67%) (P<0.05); after 2 weeks of treatment, the scores of shortness of breath, asthenia and mental fatigue in the ob?servation group were lower than those in the control group (P<0.05); after 2 weeks of treatment in the observation group, CD3+ and CD4+were higher than the control group, and CD8+ was lower than the control group (P<0.05); the serum TGF-β1 and TNF-α levels in the ob?servation group were lower than those in the control group after 2 weeks of treatment (P<0.05); after 2 weeks of treatment in the observa?tion group, the scores of physical fatigue, emotional fatigue, behavioral fatigue, cognitive fatigue and total fatigue were lower than those in the control group (P<0.05); compared with the control group (16.67%), the incidence of adverse reactions in the observation group(12.96%) was not statistically significant (P>0.05).Conclusion Shenqi Fuzheng injection in the treatment of CDF can enhance the cel?lular immune function, effectively improve the clinical symptoms of patients, improve the therapeutic effect, safe and reliable, and its anti-CRF molecular level mechanism may be related to the reduction of TGF-β1 and TNF-α expression. |
