      目的探讨体质量指数(BMI)对胃癌病人远期全因死亡风险的剂量效应。方法回顾性收集2010年1月至2011年5月在安徽医科大学第一附属医院接受根治性胃癌切除术病人的资料。分析方法采用限制性立方样条多因素Cox比例风险回归模型。结果共有593例病人纳入此项研究,BMI(22.0±3.2)kg/m2,其中BMI正常者占总人数的72.2%。在限制性立方样条模型中,当BMI=25 kg/m2时,病人的全因死亡风险为0.87[95%CI:(0.60,1.27)],差异无统计学意义(P=0.466);当BMI=30 kg/m2时,病人的全因死亡风险为0.85[95%CI:(0.35,2.03)],差异无统计学意义(P=0.587)。根据TNM肿瘤分期将胃癌病人分为Ⅰ/Ⅱ期和Ⅲ期两组,进行亚组分析,结果表明,BMI对于胃癌Ⅲ期病人远期全因死亡风险也无影响。结论BMI可能对于胃癌病人远期全因死亡风险无影响。

      Objective To investigate the dose effect of body mass index (BMI) on the long-term risk of all-cause mortality in gastric cancer (GC) patients.Methods The data of patients who underwent radical gastric cancer resection at the First Affiliated Hospital of Anhui Medical University from January 2010 to May 2011 were retrospectively collected. The analysis method used a restricted cubic spline multivariate Cox proportional hazards regression model.Results A total of 593 patients were included in this study with a BMI of (22.0±3.2) kg/m2, of whom 72.2% had a normal BMI. In the restricted cubic spline model, the risk of all-cause mortality was 0.87[95% CI: (0.60, 1.27)] when BMI = 25 kg/m2, and the difference was not statistically significant (P = 0.466). When BMI = 30 kg/m2,the risk of all-cause mortality was 0.85 [95% CI: (0.35, 2.03)], and the difference was not statistically significant (P = 0.587). Gastric cancer patients were divided into two groups according to TNM tumor stage, stage Ⅰ/Ⅱ and Ⅲ, and subgroup analysis showed that BMI had no effect on the risk of long-term all-cause mortality in patients with stage Ⅲ gastric cancer.Conclusion BMI may have no effect on the long-term risk of all-cause mortality in gastric cancer patients.
