      目的探究微小RNA(miR)-29a和内皮联蛋白在不同程度子痫前期孕妇胎盘组织中的表达水平和临床意义。方法抽签选取2017年7月至2019年7月宜昌市妇幼保健院收治的行剖宫产的子痫前期病人107例,其中轻度子痫前期组48例,重度子痫前期组59例。另选取同期行剖宫产分娩的健康孕妇56例作为对照组。分别用实时荧光定量逆转录聚合酶链反应(qRT-PCR)和免疫组织化学检测胎盘中miR-29a和内皮联蛋白表达水平,并通过Pearson相关性分析检验miR-29a和内皮联蛋白在子痫前期胎盘组织中的相关性,采用logistic回归分析影响子痫前期的危险因素。结果对照组收缩压、舒张压、平均动脉压、24 h尿蛋白比轻度子痫前期组和重度子痫前期组低,轻度子痫前期组比重度子痫前期组低(P<0.05)。对照组、轻度子痫前期组、重度子痫前期组胎盘组织miR-29a(1.09±0.21、3.18±1.19、6.04±1.91)和内皮联蛋白(0.22±0.10、0.37±0.14、0.49±0.22)表达水平依次升高(P<0.05)。胎盘组织miR-29a表达水平与内皮联蛋白表达水平呈正相关(r=0.52,P<0.001)。重度子痫前期组胎儿窘迫、产后大出血、胎盘早剥等并发症发生率均高于轻度子痫前期组和对照组(P<0.05)。二元logistic回归分析显示,miR-29a、内皮联蛋白水平偏高是孕妇发生子痫前期的独立危险因素(P<0.05)。结论子痫前期孕妇胎盘组织中miR-29a和内皮联蛋白随着病情的加重而不断升高,miR-29a和内皮联蛋白水平呈正相关,是孕妇发生子痫前期的独立危险因素。二者可作为子痫前期病情的参考指标。

      Objective To explore the expression levels and clinical significance of microRNA (miR)-29a and endoglin in placenta tissue of pregnant women with different degrees of preeclampsia.Methods A total of 107 patients with preeclampsia who underwent cesarean section in Yichang Maternal and Child Health Hospital from July 2017 to July 2019 were selected by simple random lot, including 48 cases in mild preeclampsia group and 59 cases in severe preeclampsia group. Another 56 healthy pregnant women who had caesarean section at the same time were selected as the control group. The expression levels of miR-29a and endoglin in placenta were detected by real-time quantitative PCR (qRT-PCR) and immunohistochemistry, and the correlation between miR-29a and endoglin in preeclampsia placenta was analyzed by Pearson method, logistic regression analysis was used to analyze the risk factors of preeclampsia.Results The systolic blood pressure, diastolic blood pressure, mean arterial pressure and 24-hour urine protein in the control group were lower than those in the mild preeclampsia group and the severe preeclampsia group, and the those in the mild preeclampsia group were lower than those in the severe preeclampsia group (P<0.05). The expression levels of miR-29a (1.09±0.21, 3.18±1.19, 6.04±1.91) and endoglin (0.22±0.10, 0.37±0.14, 0.49±0.22) in placenta tissue of control group, mild preeclampsia group and severe preeclampsia group increased in turn, and the differences were statistically significant (P<0.05). The expression level of miR-29a in placenta tissue was positively correlated with that of endoglin (r=0.52, P<0.001). The incidence rates of fetal distress, postpartum hemorrhage,placental abruption and other complications in severe preeclampsia group were higher than those in mild preeclampsia group and control group (P<0.05). Binary logistic regression analysis showed that high levels of miR-29a and endoglin were independent risk factors for preeclampsia in pregnant women (P<0.05).Conclusions The levels of miR-29a and endoglin in placenta tissue of pregnant women with preeclampsia increase with the aggravation of the disease. The levels of miR-29a and endoglin are positively correlated,which are independent risk factors of preeclampsia in pregnant women. Both of them can be used as reference indexes of preeclampsia.
