      目的探讨中心性肥胖 2型糖尿病( T2DM)病人血清微 RNA-221(miR-221)表达与微血管并发症的关系。方法将 2019年 6月至 2021年 9月聊城市第二人民医院收治的中心性肥胖 T2DM病人 189例作为观察组,另选取 181例健康体检者作为对照组,检测并比较两组的血清 miR-221表达水平。根据观察组有无并发微血管并发症将其分为并发组和未并发组,采用多因素 logistic回归分析微血管并发症发生的影响因素,并绘制受试者操作特征(ROC)曲线分析相关血清指标对微血管并发症发生的诊断价值。结果观察组腹围、身体质量指数和空腹血糖、糖化血红蛋白( HbA1c)、超敏 C反应蛋白( hs-CRP)、血尿酸(UA)均高于对照组( P<0.05),血清 miR-221水平高于对照组( 0.86±0.27比 0.32±0.06,P<0.05);观察组微血管并发症发生率为51.32%(97/189);并发组年龄、合并高血压及高脂血症占比、病程、未按时服药占比及空腹血糖、 HbA1c、hs-CRP、UA均高于未并发组( P<0.05)血清 miR-221水平高于未并发组( 0.91±0.25比 0.81±0.16,P<0.05)且均是中心性肥胖 T2DM病人出现微血管并发症的危险P<0.05);血清 miR-221水平诊断中心性肥胖 T2DM病人微血管症的临界值、灵敏度、特异度、曲线下面积分别为 0.83、88.66%、79.35%、0.81,其诊断价值低于 HbA1c水平( P<0.05),高于 hs-CRP和 UA水平( P<0.05),与空腹血糖水平比较差异无统计学意义( P>0.05)。结论血清 miR-221水平在中心性肥胖 T2DM病人中呈高表达,是微血管并发症发生的危险因素,且对微血管并发症发生具有重要的临床诊断价值。

      Objective To investigate the relationship between the expression of serum microRNA -221 (miR-221) and microvascular complications in patients with central obesity type 2 diabetes mellitus (T2DM).Methods One hundred and eighty-nine patients withcentral obesity T2DM treated in Liaocheng Second People's Hospital from June 2019 to September 2021 were selected as the observa-tion group and 181 healthy people were selected as the control group, and the expression levels of serum miR-221 were detected and compared between the two groups. According to the occurrence of microvascular complications in central obesity T2DM patients, theywere assigned into concurrent group and non concurrent group. The influencing factors of microvascular complications were analyzed bymultivariate logistic regression, and the receiver operating characteristic curve (ROC) was drawn to analyze the diagnostic value of rele-vant serum indicators for microvascular complications.Results The abdominal circumference, body mass index, fasting blood glucose, glycosylated hemoglobin (HbA1c), high-sensitivity C-reactive protein (hs-CRP), uric acid (UA) and serum miR-221 expression (0.86±
