      目的探讨血清白细胞介素( IL)-20水平与白内障超声乳化术后干眼症发生的相关性。方法以 2019年 8月至 2022年 8月在大同市第五人民医院行白内障超声乳化手术的 60例病人为研究组,根据术后 2周有无干眼症分为干眼组与非干眼组,并对两组的一般资料进行分析;采用酶联免疫法测定血清 IL-20表达水平;采用角膜上皮荧光素染色( CFL)、泪膜破裂时间(BUT)试验来记录各组 CFL积分及 BUT;分析病人血清 IL-20和 BUT、CFL积分的相关性; logistic回归分析白内障超声乳化术后发生干眼症的影响因素。结果非干眼组和干眼组病人眼角膜疾病史、睑板腺功能障碍差异有统计学意义( P<0.05)。与术前相比,干眼组 IL-20(19.72±4.53比 26.38±5.07)、 CFL积分在术后明显升高( P<0.05),BUT(9.70±1.37比 4.45±0.66)在术后明显减少( P<0.05)而非干眼组 IL-20、BUT及 CFL积分在手术前、后均差异无统计学意义( P>0.05)。相关性分析结果显示,白内障超声乳化术后干,眼症人血清 IL-20与 BUT呈负相关( r=?0.54,P=0.003),IL-20与 CFL积分呈正相关( r=0.45,P=0.015)。 logistic分析结果显示,血清 IL-20[OR=1.31,95%CI:(1.05,1.64)]、 BUT[OR=0.23,95%CI:(0.08,0.66)]均是白内障超声乳化术后发生干眼症的影响因素( P<0.05)。结论血清 IL-20在白内障超声乳化术后呈高表达,并与术后发生干眼症呈正相关,血清 IL-20参与该疾病的发生发展,有望成为治疗该疾病的潜在血清学指标。

      Objective To investigate the correlation between serum interleukin (IL)-20 level and dry eye after phacoemulsification. Methods A total of 60 patients who underwent phacoemulsification in Datong No.5 People's Hospital from August 2019 to August2022 were taken as the research subjects and assigned into dry eye group and non-dry eye group according to whether there was dry eye2 weeks after operation, and the general data of the two groups were analyzed. The expression level of serum IL-20 was measured by ELISA. The corneal epithelial fluorescein staining (CFL) and tear break-up time (BUT) test were used to record the CFL scores and BUT of each group. The correlation between serum IL-20, BUT and CFL scores was analyzed. Logistic regression analysis was made ofthe influencing factors for dry eye after phacoemulsification.Results There were significant differences between the non-dry eye groupand the dry eye group in the history of corneal diseases and meibomian gland dysfunction. Compared with preoperation, the IL-20 (19.72±4.53 vs. 26.38±5.07) and CFL score in the dry eye group increased greatly (P<0.05), and BUT (9.70±1.37 vs. 4.45±0.66) de? creased greatly after operation (P<0.05), while IL-20, BUT and CFL score in the non-dry eye group had no great difference before and after operation (P>0.05). The results of correlation analysis showed that serum IL-20 was negatively correlated with BUT (r=?0.54, P= 0.003), and IL-20 was positively correlated with CFL score (r=0.45, P=0.015). Logistic analysis results showed that serum IL-20 [(OR= 1.31, 95%CI:(1.05,1.64)], and BUT [(OR=0.23, 95%CI:(0.08,0.66)] were both the influencing factors of dry eye after phacoemulsifica? tion (P<0.05).Conclusions Serum IL-20 is highly expressed after phacoemulsification, and is positively correlated with dry eye after surgery. Serum IL-20 is involved in the occurrence and development of the disease, which is expected to become a potential serologicalindex for the treatment of the disease.
