      目的分析止咳平喘十二味合剂联合乌苯美司胶囊对急性加重期慢性阻塞性肺疾病(慢阻肺)病人氧合指数、免疫失衡及炎性因子的影响。方法选取铜川市人民医院 2019年 1月至 2020年 3月收治的 60例急性加重期慢阻肺病人作为研究对象。按随机数字表法分为两组,对照组 30例,采用常规治疗 +乌苯美司胶囊口服( 3次/天, 10毫克 /次);观察组 30例,在对照组基础上加用止咳平喘十二味合剂( 2次/天, 1袋/次,餐后 30 min口服)。两组均连续治疗 8周。观察并记录两组治疗前后氧合指数、慢阻肺症状(CAT)评分、 6 min步行试验( 6MWD)、免疫功能指标(包括 CD3+CD4+CD8+和 CD4+/CD8+)、炎性因子[白细胞介素 -8(IL-8)、肿瘤坏死因子α(TNF-α)、C反应蛋白(CRP)]及肺功能指标[包括、用力肺、活量( FVC)、第 1秒用力呼气容积(FEV1)及 FEV1/FVC]然后进行统计学分析。结果两组治疗后氧合指数、 CAT评分及 6MWD均明显改善,但观察组显著更优(P<0.05)其中对照组治,疗后氧合指数为( 221.77±20.76)观察组为( 245.65±19.76);两组治疗后 CD3+CD4+和 CD4+/CD8+明显+,升高, CD8明显下降,但观察组明显优于对照组( P<0.05),其中对照组治疗后 CD3+、CD4+/CD8+分别为(6、0.71±4.82)%、(0.97±

      Objective To analyze the effect of Zhike Pingchuan twelve flavor mixture combined with ubenimex capsule on oxygen.ation index, immune imbalance and inflammatory factors in patients with COPD during acute exacerbation.Methods Sixty patientswith acute exacerbation of COPD who were admitted to Tongchuan People′s Hospital from January 2019 to March 2020 were selectedas the research objects. According to the random number table method, they were assigned into two groups: 30 cases in the controlgroup, orally administered with conventional treatment + ubenimex capsules (t.i.d, 10 mg/time), and 30 cases in the observation group,administered additionally with Zhike Pingchuan twelve mixture (twice a day, a pack/time, taking orally half an hour after meals) . Bothgroups underwent treatment for 8 weeks. We Observed and recorded the oxygenation index (OI), COPD assessment test (CAT) score, 6-minute walking test (6-MWD), immune function indexes (including CD3+, CD4+, CD8+ and CD4+/CD8+), inflammatory factors [interleu. kin-8 (IL-8), tumor necrosis factor alpha (TNF-α), C-reactive protein (CRP)] and lung function indicators [including forced vital capaci.ty (FVC), forced expiratory volume in one second (FEV1) and FEV1/FVC], and then statistical analysis was made.Results After treat. ment, the OI, CAT score and 6MWD were significantly improved, which were significantly better in the observation group (P<0.05).The OI in the control group and the observation group were (221.77±20.76) and (245.65±19.76), respectively. After treatment, CD3+,CD4+ and CD4+/CD8+ increased significantly and CD8+ decreased significantly in both groups, but the observation group was significant. ly better than the control group [CD3+:(0.97±0.34) vs. (1.38±0.23), CD4+/CD8+: (60.71±4.82)% vs. (67.75±4.71)%; P<0.05]. After treat. ment, IL-8, TNF-α and CRP were significantly decreased in both groups, and IL-8 [(17.63±4.01) ng/L], TNF-α [(38.59±6.13) ng/L] and CRP [(15.14±2.24) mg/L] in the observation group were significantly lower than those in the control group [(22.87±4.46) ng/L, (49.55±6.57) ng/L, (30.22±6.37) mg/L, respectively; P<0.05]. FVC, FEV1 and FEV1/FVC increased significantly after treatment in the two groups, but the indicators in the observation group were significantly better (P<0.05).Conclusion The combination of Zhike Pingch. uan twelve mixture and ubenimex capsule can effectively optimize the oxygenation index, immune function and inflammatory responsein patients with COPD during acute exacerbation, improve the clinical symptoms, lung function and motor function of patients,which isworthy of clinical promotion.
