      目的 探讨超声刀联合腋窝处皮肤外固定用于预防乳腺癌改良根治术后皮下积液的应用价值。方法 选择2014年8月至2016年1月期间于黄冈市中心医院行乳腺癌改良根治术治疗的120例患者纳入临床研究,根据其入院检查时间编号并按单双号分作两组,其中单号60例为对照组采取电刀联合腋窝处皮肤外固定治疗,双号60例为研究组采取超声刀联合腋窝处皮肤外固定,对比手术疗效差异及术后皮下积液发生情况。结果 研究组患者手术时间(108.45±5.87) min、术中出血量(54.67±4.16) mL、淋巴结清扫数目(20.43±1.72)枚,平均住院时间(11.66±1.77) d,与对照组比较均明显减少(t=31.805,33.806,4.832,13.826,P<0.05)。研究组患者术后总引流量(357.65±18.54) mL、引流管拔除时间(8.48±0.65) d,与对照组比较均明显减少(t=29.478,27.581,P<0.05)。研究组患者术中副损伤0.00%、皮下积液3.33%,与对照组比较均明显降低(χ2=4.386,3.927,P<0.05);而两组术后再出血、游离皮瓣坏死发生率比较差异无统计学意义(χ2=0.000,0.000,P>0.05)。结论 超声刀联合腋窝处皮肤外固定用于乳腺癌改良根治术较传统电刀治疗效果显著,可减少术中出血、术后引流量,缩短手术时间、引流管拔除时间及住院时间,能预防术后皮下积液的发生,利于皮瓣愈合,安全、可行,值得推荐。

      Objective To explore the value of ultrasonic knife combined with axillary skin external fixation on prevention of subcutaneous effusion after modified radical mastectomy of breast cancer.Methods One hundred and twenty cases of breast cancer treated by modified radical mastectomy in Huanggang Central Hospital from August 2014 to January 2016 were included in the clinical study,and were assigned into two groups by odd-even number according to the serial number of patient admission examination time.The single number of 60 patients in the control group were taken the electric knife combined with external fixation for the treatment of armpit skin,while the double number of 60 cases in study group were taken ultrasonic scalpel combined with armpit skin external fixation;the differences in curative effect and the occurrence of postoperative subcutaneous effusion were compared.Results The mean operative time (108.45±5.87) min,the amount of blood loss (54.67±4.16) mL,the number of lymph node dissection (20.43±1.72) and the average length of hospital stay (11.66±1.77) d were significantly lower in the study group than those in the control group (t=31.805,33.806,4.832,13.826,P<0.05).The total drainage volume (357.65±18.54) mL and drainage tube drainage time (8.48±0.65) d in the study group were significantly lower than those in the control group (t=29.478,27.581,P<0.05).In the study group,the intraoperative complications and subcutaneous effusion were 0.00% and 3.33%,respectively,which were significantly lower than those in the control group (χ2=4.386,3.927,P<0.05).There were no significant differences in the incidence of postoperative rebleeding and free flap necrosis between the two groups (χ2=0.000,0.000,P>0.05).Conclusion The ultrasonic knife combined with the skin external fixation for the treatment of breast cancer is more effective than the traditional electric knife.It can reduce intraoperative bleeding and postoperative drainage volume,shorten the operation time,drainage tube extraction time and hospital stay,and prevent the occurrence of postoperative subcutaneous effusion.It is conducive to skin flap healing,safe,feasible,and is worth recommending.
