      目的 探究髋臼横断骨折四种内固定方式的臼顶接触性变化。方法 采用成人男性半骨盆标本制作髋臼横断骨折模型16个,随机分成四组,予以A组:前柱重建接骨板,B组:前柱锁定接骨板,C组:骨盆缘下重建接骨板,D组:骨盆缘下锁定接骨板。通过垂直加载至生理体质量600 N,维持1 min,用双页型压敏片依次测量完整髋臼I组和A、B、C、D四组的臼顶接触特性变化。结果 生理体质量600N下,A、B、C、D组内固定后接触特性均不能恢复至正常,与I组比较,负重面积,接触压强及峰值压强均存在统计学差异(P<0.05),同时A、B、C、D组之间比较,接触特性也存在统计学差异(P<0.05),并且接触特性依次增强。结论 在生理体重600N下,对髋臼横断骨折予以四种不同内固定,接触特性均不能恢复至正常,且骨盆缘下接骨板优于前柱接骨板,能增加臼顶负重面积,降低臼顶平均压强及峰值压强,同时锁定接骨板的接触特性强于重建接骨板。

      Objective To evaluate the changes of acetabular dome contact in four internal fixation methods for acetabular transverse fractures.Methods Acetabular transverse fracture models were created by 16 male’s hemipelvis specimens, assigned into four groups randomly.The fractures were treated respectively with the following internal fixation methods,anterior reconstruction plate (group A),anterior locking compression plate (group B),infrapectineal buttress reconstruction plate (group C),infrapectineal buttress locking compression plate (group D). Then vertical compressing loading was increased to physiological weight of 600N, and maintained 1 minute, followed by measurement of acetabular dome contacts of intact acetabulum in group Iand groups A,B,C,D with double layer films. Results Under physiological weight of 600 N, acetabular dome contacts of groups A,B,C,D failed to return to normal, and compared with group Ithere were significant differences in the loading area, the mean pressure, the peak pressure, horizontal displacements and longitudinal displacements among groups(P<0.05). Meanwhile, there were significant difference in contact characteristics among groups A,B,C,D(P<0.05), and the contact characteristics were enhanced in order.Conclusions Under physiological weight of 600 N, contact characteristics of four respective internal fixations all could not restore to the normal. Infrapectineal buttress plate was better than anterior plate, which widened acetabular dome loading area, reduced acetabular dome mean pressure and peak pressure.Contact characteristics of locking plate were stronger than those of reconstruction plate.
