目的 利用二维斑点追踪技术(2D-STI)评价维持性血液透析患者左室心肌扭转运动功能及同步性的变化,探讨其应用价值。方法 收集血液净化中心经动静脉内瘘(AVF)行维持性血液透析的晚期肾病患者68例,并根据肺动脉压分级标准[1]进行分组比较,另外选择性别、年龄相匹配的健康志愿者30例为对照组。双平面法测量左室射血分数(LVEF)。应用2DSTI技术分别测量左室短轴基底段扭转角度峰值(Prot-B)、心尖段扭转角度峰值(Prot-A)、整体扭转角度峰值(Ptw-All)、达峰时间(T-Ptw)及左心室同步化指数(TAS-POST)。结果 透析各组左室扭转参数与对照组比较Prot-B、Prot-A、Ptw-All减低,差异有统计学意义(P<0.05)。并且随肺动脉压增高左室整体扭转角度逐渐下降,两者呈负相关(r=-0.689,P<0.01)。无肺动脉高压组与对照组比较T-Ptw及TAS-POST差异无统计学意义,而肺动脉压增高各组与对照组及无肺动脉高压组比较T-Ptw及TAS-POST差异有统计学意义(P<0.05)。中重度肺动脉高压(PH)组与对照组、无PH组及轻度PH比较,LVEF明显减低,差异有统计学意义(P<0.05),而轻度PH组、无PH组与对照组比较LVEF差异无统计学意义。结论 维持性血液透析患者左心室心肌扭转及解旋运动功能减低,同步性下降, 2D-STI可以定量的评价左室心肌旋转和扭转的力学改变,为临床提供重要诊断信息。 |

Objective To evaluate left ventricular torsion and synchronicity function in maintenance hemodialysis patients by using two-dimentional speckle tracking imaging(2D-STI),and to discuss the application value.Methods The study objects consisted of 68 maintenance hemodialysis patients who were created arteriovenous fistula(AVF) in Blood Purification Centre of Bozhou People’s Hospital, who were assigned in different groups according to the pulmonary artery pressure(PH) group classification standard,and 30 healthy volunteers were chosen as control group. Left ventricular ejection fraction(LVEF) was measured by bi-Simpson’s method in all study objects. Left ventricular short axis basal segment torsion angle peak (Prot-B), apex section torsion angle peak (Prot-A), the overall torsion angle peak (Ptw-All), tmax (T-Ptw) and left ventricular synchronization index (TAS-POST) were measured by 2DSTI.Results Compared with control group, Prot-B, Prot-A, Ptw-All decreased in patients of different hemodialysis groups with statistically significant difference(P<0.05). With the severity of PH, Ptw-All decreased progressively.Correlation analysis showed that Ptw-All was negatively correlated with pulmonary arterial pressure (r=-0.689,P<0.01). Compared with control group and no pulmonary hypertension group, Differences in T-Ptw and TAS-POST were not statistically significant between control group and no pulmonary hypertension group, while statistically significant among pulmonary hypertension groups(P<0.05).LVEF decreased significantly in moderate and severe PH group compared with control group,no-PH group and mild PH group(P<0.05). Difference in LVEF was not significant among control group,no-PH group and mild PH group.Conclusions Pulmonary arterial pressure was negatively correlated with myocardia Ptw-All.2D-STI can evaluate left ventricular myocardial torsion function and synchronicity quantitatively, which can provide important diagnostic information. |
